Abstract [sv]

Bakgrund: Behovet av palliativ vård är stort i hela världen och cirka 40 miljoner människor behöver vårdas palliativt varje år. Många människor önskar att få dö i sina egna hem och därför har palliativ vård i hemmet blivit en växande vårdform som är under utveckling. Palliativ vård grundar sig på de fyra hörnstenarna: symtomlindring, teamarbete, kommunikation och relation samt stöd till de närstående. Dessa bygger på att lindra och förebygga lidandet hos patienten. Sjuksköterskan har en ledande roll i omvårdnaden och ska arbeta genom ett etiskt och personcentrerat förhållningssätt. Sjuksköterskans arbete grundar sig i respekt för patientens integritet och fokusering på att främja hälsa, lindra symtom och lindra lidandet.

Syfte: Syftet var att belysa sjuksköterskors erfarenheter av att ge palliativ vård i patientens egna hem.

Metod: En litteraturöversikt gjordes enligt Fribergs (2017) metod. De databaser som användes inför sökningen av vetenskapliga artiklar var följande: CINAHL Complete, PubMed och Medline. I arbetet inkluderades nio kvalitativa och en kvantitativ artikel, då dessa bedömdes svara på syftet att belysa sjuksköterskors erfarenheter av palliativ vård i hemmet. Utvalda artiklar analyserades med hjälp av Fribergs analysmetod.

Resultat: Resultatet presenteras i fyra teman: Vårdrelationens betydelse, Arbetsgruppens betydelse, Behovet av kompetensutveckling och Personlig påverkan. Sjuksköterskorna hade positiva och negativa erfarenheter av den palliativa hemsjukvården. Goda vårdrelationer, fungerande samarbeten och den tillfredsställelse som arbetet gav upplevdes som positivt. Otillräcklig kunskap, brist på tid samt svårigheter i att möta etiska dilemman upplevdes som negativt.

Diskussion: I metoddiskussionen diskuterar författarna styrkor såväl som svagheter med litteraturöversikten. I resultatdiskussionen diskuteras huvudfynden i resultatet genom Katie Erikssons omvårdnadsteori, tidigare forskning, bakgrund och författarnas egna reflektioner.

Abstract [en]

Background: Worldwide, approximately 40 million people are in need of palliative care every year. Many people wishes to die in their own homes, palliative home care is therefore a growing form of care. Palliative care is based on the four cornerstones: relieve symptoms, teamwork, communication and relations and family support, which aims to alleviate and prevent patient suffering. The nurse has a leading role in nursing and shall work through an ethical and person-centered approach. The nurse's work is based on respect for the patient's integrity and focus on promoting health, alleviating symptoms and suffering.

Aim: The aim of the study was to illuminate nurses’ experiences of palliative care in the patient's own home.

Method: A literature review was made according to Friberg's (2017) method. The databases that were used for the search of scientific articles were as follows: CINAHL Complete, PubMed and Medline. Nine qualitative and one quantitative article were included, since they were assessed to respond to the aim of exploring nurses' experiences of palliative care at home. Selected articles were analyzed using Friberg's method of analysis.

Results: The results are presented in four themes: The importance of a care relationship, The importance of colleagues, The need of education and Personal impact. The nurses had positive and negative experiences of palliative home care. Good care relationships, good cooperation in the team and satisfaction through work were perceived as positive. Insufficient knowledge, lack of time and difficulties in meeting ethical dilemmas were on the other hand perceived as challenging.

Discussion: In the method discussion, the authors discusses strengths as well as weaknesses with the literature review. In the result discussion, the main findings in the results are discussed through Katie Eriksson's nursing theory, previous research, background and the authors' own reflections.
