Abstract [sv]

Bakgrund: Tidigare forskning visar att många patienter upplever en låg grad av delaktighet i vården. Som specialistsjuksköterska ingår det att lyfta patientens perspektiv. Enligt lag ska patientens självbestämmande respekteras och patienten ska få information om vård och behandlingsmetoder. Delat beslutsfattande är en metod som kan användas för att inkludera patienter i psykiatrisk vård.

Syfte: Syftet var att beskriva patientens syn på delat beslutsfattande inom psykiatrisk vård.

Metod: Metoden var en litteraturöversikt med systematiskt tillvägagångssätt baserad på kvalitativa studier. Litteratursökningen genomfördes i tre akademiska databaser och totalt inkluderades elva vetenskapliga originalartiklar.

Resultat: Två teman och fyra subteman framkom i resultatet. Det första temat var Faktorer som möjliggör delat beslutsfattande, med subtemana Relationen till vårdpersonalen, Vikten av informationsutbyte och feed-back samt Delat ansvar och att fatta beslut. Det andra temat var Hindrande faktorer för delaktighet med subtemana Maktlöshet och utanförskap, Ogynnsamt bemötande och bristfällig information samt Bristande resurser.

Slutsats: Många patienterna upplevde att de inte var tillräckligt delaktiga i sin vård. Det framkom att delat beslutsfattande är en metod som kan skapa delaktighet genom att främja tillit, utbyte av information och transparent dialog där båda parter ses som experter och vars kunskap värderas lika. Genom delat beslutsfattande befinner sig patienten i beslutsprocessen istället för att stå utanför. Föreliggande litteraturöversikt stödjer att delat beslutsfattande kan vara värdefullt inom psykiatrisk vård, där relationen och dialogen är viktiga verktyg för specialistsjuksköterskan.

Abstract [en]

Background: Previous research shows that many patients experience a low degree of participation in care. As a specialist nurse, it is important to raise the patient's perspective. By law, the patient's self-determination must be respected and the patient must receive information about care and treatment methods. Shared decision making is a method that can be used to include patients in psychiatric care.

Aim: The purpose was to describe the patient's view of shared decision-making in psychiatric care.

Method: The method was a literature review with a systematic approach based on qualitative studies. The literature search was conducted in three academic databases and a total of eleven original scientific articles were included.

Results: Two themes and four subthemes emerged in the result. The first theme was Factors that enable shared decision-making, with the subthemes Relationship to healthcare staff, The importance of information exchange and feedback and Shared responsibility and taking a decision. The second theme was Obstacles to participation, with the subthemes Powerlessness and exclusion, Unfavorable treatment and lack of information and Lack of resources.

Conclusion: Many patients felt that they were not sufficiently involved in their care. It emerged that shared decision-making is a method that can create participation by promoting trust, exchange of information and transparent dialogue where both parties are seen as experts and whose knowledge is valued equally. Through shared decision-making, the patient is in the decision-making process instead of standing outside. The present literature review supports that shared decision-making can be valuable in psychiatric care, where the relationship and dialogue are important tools for the specialist nurse.
