Abstract [sv]

Bakgrund: I Sverige avlider årligen 90 000 personer och cirka 75 000 beräknas ha palliativa vårdbehov inom slutenvården eller på vård- och omsorgsboende. Anhöriga har en viktig roll i omvårdnaden vid palliativ vård. Sjuksköterskan har ett ansvar att ge stöd och information.

Syfte: Att belysa anhörigas upplevelser av palliativ vård i livets slut för äldre personer på vård-och omsorgsboende.

Metod: En litteraturöversikt som omfattar tio kvalitativa vetenskapliga artiklar. Informationssökning utfördes i två databaser PubMed och CINAHL Complete. Artiklarna granskades enligt Fribergs beskrivning. 

Resultat: Resultatet redovisas i fyra kategorier: Att vara delaktig, En god vårdrelation, Att känna misstro till vården, Lidande och sorg. Anhörigas upplevelser var både positiva och negativa i vården vid livets slut. Anhöriga ville ha regelbunden information och samtal med vårdpersonalen för att känna trygghet och vara delaktiga i beslutfattande i vården vid livets slut. 

Sammanfattning: För att främja äldre personers livskvalitet engagerade sig anhöriga i deras närståendes vård i livets slut.  Kommunikation och information påverkade deras upplevelser av vården och de ville bli lyssnade till och sedda. Anhörigas delaktighet i vården vid livets slut kan bidra till en god vårdkvalitet. Bättre information och stöd från sjuksköterskor kan öka anhörigas delaktighet. Avsaknaden av kommunikation, information och stöd resulterade känslor av otrygghet, sårbarhet och misstro till vården vid livet slut.

Abstract [en]

Background: Approximately 90,000 people die yearly and about 75,000 people estimates to have palliative care in nursing homes in Sweden. The relatives have an important role for the patients in palliative care. The nurses have a responsibility to provide support and give information to both relatives and patient in the end-of-life care

Aim: The object of this paper was to describe the relatives' experiences of end-of-life care for older people in nursing homes.

Method: A literature review comprising ten qualitative scientific articles. Information retrieval was performed in two databases PubMed and CINAHL Complete. The articles were reviewed according to Friberg's description

Results: The results reported four categories: experiences of being involved, a good care relationship, feeling distrust of care, suffering and sorrow. Relatives’ experiences were both negative and positive of palliative care. Relatives wanted regular basis communication, information, and conversation with the respective care staff for felling safe and most of the relatives’ wanted to be involved with decision making in the end-of-life.

Summary: Relatives would participate in the care of their next-of-kin at the end-of -life to develop their quality of life. The family caregivers felt that communication and information affected their experience, and this enhanced a demand to be listened and to be seen. Relatives involvement in end-of-life care contribute to a good care. Better information and support from the nurses increase the involvement of the relatives. The cause of insufficiency of communication,   information and support leads to insecurity, vulnerability, and mistrust in the end-of-life care.
