Abstract [sv]

Bakgrund: Substansbrukssyndrom är en sjukdom med ett förlopp som innefattar både en fysisk och psykisk påfrestning. En koppling mellan psykisk ohälsa och substansbrukssyndrom har påvisats och det anses ha avsevärd negativ påverkan på individens liv. På grund av att substansbrukssyndrom är ett folkhälsoproblem som kan leda till förkortad livslängd, så har det skapats strategier för att minska problematiken. Den specifika kompetensen för sjuksköterskan är omvårdnad. Detta innebär att sjuksköterskan ska ha ett etiskt förhållningssätt till patienter och utöva sitt yrke utifrån det. Sjuksköterskans bemötande av patienter med substansbrukssyndrom är viktig i sammanhanget. Men trots det präglas bemötandet alltför ofta av okunskap om substansbrukssyndrom och om substansbrukets implikationer.

Syfte: Syftet var att beskriva upplevelser hos personer med substansbrukssyndrom i mötet med vårdpersonal inom hälso- och sjukvård.

Metod: Litteraturöversikt som grundar sig i tio vetenskapliga artiklar. Samtliga artiklar hade en kvalitativ ansats. Utvalda artiklar analyserades enligt Fribergs metodbeskrivning och beroende på de teman som identifierades i artiklarna, så delades de in i respektive huvudkategorier och underkategorier.

Resultat: Patienters upplevelser uttrycktes framför allt i känslor av stigmatisering. Patienternas erfarenheter visade att de upplevde särbehandling på grund av deras syndrom. Dessa upplevelser ledde till fördröjd vårdkontakt eftersom patienterna utvecklade motstånd mot att söka vård. Faktorer som påverkade patienternas upplevelser negativt grundades i relationen till sjuksköterskor, som kännetecknades av negativt bemötande och brist på kunskap kring diagnosen substansbrukssyndrom. Positiva upplevelser förekom i mindre utsträckning, dessa uppvisades oftast av god kommunikationsförmåga från sjuksköterskans sida samt sjuksköterskans höga kunskapsnivå.

Sammanfattning: Litteraturöversikten visar att relativt många individer med substansbrukssyndrom upplever svårigheter i kontakt med vården i form av otrygghet och avvisning. Detta har visats grunda sig ofta i okunskap om och brist av erfarenhet av patienter med substansbrukssyndrom hos sjuksköterskan. Patienternas positiva upplevelser förefaller vara viktiga att lyftas fram som exempel på de förbättringar som kan göras i en kontext av den undersökta problematiken.

Abstract [en]

Background: Substance use syndrome is a disease that includes both physical and mental stress. A connection between mental illness and substance use syndrome has been demonstrated, which is considered to have a strong impact on a person's life. The substance use syndrome is classified as a  public health problem that can lead to shortened lifespan; therefore, different strategies have been outlined to reduce the effects of mentioned problem. The specific competence of the nurse is nursing. This means that the nurse must have an ethically grounded approach to patients and, subsequently, to practice her profession based on that. The nurse's  treatment and approach of patients with substance use syndrome is of great importance. Despite this, the mentioned approach is often characterized by ignorance about the patient’s substance  addiction and about the implications of that addiction

Aim: The aim of this study is to describe the experiences of patients with substance use syndrome in their encounter with nurses within the health care system.

Method: Literature review based on ten scientific articles. All articles had a qualitative approach. Selected articles were analyzed according to Friberg's method description and depending on the themes identified in the articles, they were divided into respective main categories and subcategories.

Results: Patients' experiences were expressed mainly in terms of experiencing stigmatization. The patients' experiences indicate that they have been treated differently because of their syndrome. This led to delayed care contact as the patients developed a resistance towards seeking public health care. Factors that affected patients' experiences negatively were based on their encounter with nurses, which was characterized by negative treatment and a lack of knowledge about the diagnosis of substance use syndrome. Positive experiences occurred to a lesser extent, but these were usually characterized by good communication skills on the part of the nurse and the nurse's high level of knowledge.

Summary: This literature review shows that relatively many individuals with substance use syndrome experience difficulties in contact with public health care in forms of insecurity and rejection. Mentioned difficulties seem to be often based on the nurse's ignorance of and lack of experience with patients with substance use syndrome. The patients' positive experiences appear to be important as examples of improvements that can be made in the context of the investigated issue.
