Abstract [sv]

Bakgrund: Etisk stress innebär att uppleva stress som är relaterad till etiska utmaningar. Inom geriatrisk vård finns även patienter som vårdas i livets slutskede i behov av palliativ vård. Palliativ vård är ett begrepp som präglas av en helhetssyn på människan. Sjuksköterskorna i den geriatriska vården möter etiska utmaningar, då de bör ta hänsyn till ett helhetssyn samtidigt som det finns varierande typ av patienter och varierande uppgifter som alla kräver sjuksköterskans närvaro, vilket kan leda till upplevelser av etisk stress.

Syfte: Syftet med studien var att beskriva sjuksköterskors upplevelser av etisk stress vid vård av patienter i livets slutskede på en geriatrisk vårdavdelning.

Metod: En kvalitativ metod har använts. Data samlades in genom enskilda intervjuer med nio sjuksköterskor verksamma på en geriatrisk vårdavdelning. Intervjuerna har sedan analyserats utifrån en kvalitativ innehållsanalys.

Resultat: Fyra kategorier bildades som beskriver upplevelsen av etisk stress. Dessa kategorier är: känslor av otillräcklighet och misslyckande, känslor av maktlöshet och frustration, bristande kunskap och erfarenhet, bristande samsyn mellan vårdpersonal och närstående.

Diskussion: Resultatet överensstämmer med tidigare forskning om etisk stress. God kommunikation förbättrar samarbetet mellan olika personalkategorier, vilket leder till god vård och ökad vårdkvalitet. Vidare diskuteras sjusköterskornas begränsade handlingsutrymme, behov av att träna förmåga till att se realistiskt på vad hon/jan har för ansvar och betydelsen av utbildning och erfarenhet av palliativ vård.

Abstract [en]

Background: Ethical stress implies stress related to ethical challenges. In geriatric care there are also patients in their end of life, requiring palliative care. Palliative care is a term characterized by a comprehensive view of the human being. Geriatric nurse’s encounter ethical challenges where they need to take in to consideration both this comprehensive view alongside other factors and tasks, all demanding their presence and attention. This could ultimately lead to ethical stress.

Aim: The aim was to describe nurses' experiences with ethical stress in care for patients in palliative care in a geriatric ward.

Methods: A qualitative approach was used. Data were collected through individual interviews with nine nurses working in a geriatric ward. The interviews were then analyzed based on a qualitative content analysis.

Results: Four categories were formed which describes the experience of ethical stress. These categories are: feelings of inadequacy and failure, feelings of powerlessness and frustration and lack of knowledge and experience and a lack of consensus between health professionals and relatives.

Discussions: The result concurs with previous research about ethical stress. Good communication improves the cooperation between different personnel categories, which leads to better quality of care as well as care in general. The conclusion is, further discussions about the nurse’s restricted room for action, the need to further improve the ability to understand their responsibilities including the meaning and experience of palliative care.
