Elderly peoples' experience of nursing care after a stroke: from a gender perspective. Aim. This paper is a report of a study conducted to explore, from a gender perspective, older people's experiences of nursing care after a stroke. Background. Little attention has been given to gender differences in nursing care. The majority of people who have a stroke are older. Improving knowledge of the gender perspectives of older men and women regarding nursing care after stroke is crucial. Method. This was a qualitative study based on interviews with five women and five men between 66 and 75 years of age, who had received nursing care at a ward for stroke rehabilitation. The data were collected in 2006. Qualitative content analysis of the interviews was carried out. Findings. A main theme and five categories, all common to both men and women, were identified. The main theme, to promote recovery of the body, encompassed the categories. There were, however, gender differences in how the nursing care received was experienced. The ways patients experienced nursing care seemed to be linked with their lives as women and men before they had the stroke. Their perceptions are linked with their lives as women and men before they had their stroke. Both men and women will reclaim former abilities but what they perceive to be the goals of nursing care and rehabilitation may differ. Conclusion. Nurses need to increase their awareness and knowledge concerning the similarities and gender differences in the experiences and needs of older people, both men and women.

    Abstract [en]

    Introduction: The therapeutic alliance is considered an important factorregardless of therapeutic technique or theory. This essay discusses the affectivefocusedpsychotherapeutic method named Intensive short-term dynamicpsychotherapy (ISTDP) with focus on the therapeutic alliance. The aim of thisstudy is to examine how psychotherapists experience and describe the therapeuticalliance in ISTDP.

    Issues: How do psychotherapists experience and describe the therapeutic alliancein ISTDP?

    Method: The data collection method is qualitative semi-structured interviews.The material was analysed by using the processing method of empirical thematicanalysis.

    Results: The result of the interviews are first presented by the informants´descriptions of the therapeutic alliance in ISTDP, followed by five emergingthemes; Responsiveness, Tempo/Pace, Body language, Press and Skill.

    Discussion: The essay shows that the psychotherapist experience and describethe ISTDP method as a way to jointly with the patient create, a strong consciousand unconscious therapeutic alliance and a healing relationship. The result wassimilar in all respondents and consistent with theory and previous research.Possible future studies are suggested.

    Abstract [en]

    The aim of the study was to examine how children with chronic illnesses narrate their experience of being comforted in hospital. During interviews, seven children, 4-10 years old described their experiences and made drawings. Thematic content analysis revealed following themes: being physically close to one's family, feeling safe and secure, staff being there for the children, and children being there for parents and siblings. Mother was identified as the most important comforter. The findings suggest that trusting in the staff's knowledge and professional skills is a prerequisite for children to feel "at home", and safe in hospital. Being close to one's family is even more important.

    Abstract [en]

    Background: Schizophrenia is a mental disorder with a high suicidal risk. The most common symptoms experienced with schizophrenia are auditory hallucinations which can be experienced both in a positive and a negative way. The disorder is stigmatized most likely due to a lack of knowledge and a fear of the person.

    Aim: To describe adults experiences of auditory hallucinations induced by schizophrenia.

    Method: The literature review consisted of twelve scientific articles of which nine were conducted using a qualitative approach and three with a quantitative research methodology. The articles were found in the databases CINAHL Complete, PubMed and PsycINFO. The following words were used in the databases: hallucination, voice hearing, psychosis, patient, life, hearing voices, nursing, people who hear voices, auditory hallucinations, schizophrenia and experiences.

    Results: The results were divided into four themes: The nature of the voice described the characteristics of the voice i.e. tone, feeling, intensity and duration. The relation to the voice described how the affected persons identified the voice as a part of themselves. Finding meaning and coping strategies described the different strategies which were used to cope with the voices. The impact on the individual highlighted how the person was affected by the voices by an altered self, added self-harm behavior, isolation and loss of control.

    Discussions: The results were discussed with the concepts of life world and finding meaning. Finding meaning was found as one of the affected person’s most important coping strategies for dealing with the auditory hallucinations.

    Abstract [en]

    Background: Chronic obstructive pulmonary disease (COPD) is one of today's most common diseases, and is expected in less than ten years to become the third largest cause of death worldwide. The disease causes breathlessness, cough and increased sputum. Most people who contract the disease are individuals who are or have been smokers. People living with the disease often experience a hard time to not have the same strength as before and experiencing stigmatizing condemnation because the presumable cause of the disease.

    Aim: The aim with this study was to describe the experience of living with chronic obstructive pulmonary disease.

    Methods: This is a literature review based on eleven scientific articles. The articles are reviewed and quality analyzed by both the authors in several steps according to the recommendations of the analysis procedure by Friberg (2012). All the articles are available in the databases PubMed and CINAHL and are published 2008-2013.

    Results: The results proved that the physical symptoms that were most problematic for a person with the disease is to have problem with their breathing. Other symptoms recognized as troublesome was the increased mucous secretion, cough and fatigue, which are common in COPD. The psychological symptoms that were common were anxiety, depression and frustration about the loss of energy and this was mainly due to the breathing. Condemnatory attitudes about the disease's onset with questions about the past and/or current smoking behavior which created feelings of guilt and shame. Many people decided to continue smoking despite their diagnosis and feelings of guilt. They saw the smoking as a routine and an activity in their daily lives that they didn’t want to be without.

    Discussion: The discussion concluded that COPD involved a life of suffering for those who gets the disease. Suffering was experienced in loss of social interactions. According to the nursing theorist Katie Eriksson life suffering arise when energy is not sufficient to engage in social interactions. Suffering could also be experienced when a person became dependent on others to cope with everyday tasks, when the energy wasn’t enough. The physical and psychological symptoms that emerged because of the disease were experienced as a lower quality of life. Judgmental attitudes that could occur to people with COPD could cause a suffering. According to Eriksson the reason to this can be that the person does not feel valued as a person. Those who chose to continue smoking felt that smoking resulted in a quality of life and reduced suffering.

    Socialstyrelsen (2011) skriver att det är under de senaste tio åren som man har börjat uppmärksamma barn som bevittnat våld och att dessa barn kan reagera på samma sätt som barn som själva har utsatts för våld och är i behov av stöd och hjälp. Syftet med studien är att öka kunskapen om Socialtjänstens och Polisens arbete med barn som bevittnat våld. Målet är att försöka förstå om det finns en skillnad på myndigheternas bemötande med barn som har bevittnat våld gentemot barn som själva har utsatts för våld och hur denna skillnad i så fall yttrar sig. Studien har en kvalitativ ansats med semistrukturerade intervjuer. Vi genomförde tre intervjuer med Socialtjänsten och tre med Polisen. Våra tre frågeställningar är: Hur arbetar Polis respektive Socialtjänst med barn som är offer för familjevåld? Hur jobbar Socialtjänsten och Polisen med barn som bevittnat våld? På vilket sätt skiljer sig Polisens och Socialtjänstens bemötande med barn som har bevittnat våld gentemot barn som själva har utsatts för våld?

    Vi använde oss av organisationsteorin för att på så sätt kunna beskriva hur Polisen och Socialtjänsten är organiserade och hur de bemöter barn som bevittnat våld. Resultatet från vår studie visar att informanterna upplever att det finns en stor skillnad med att arbeta med barn som bevittnat våld i jämförelse med barn som själva utsatts för våld. Informanterna menar att detta beror på att barn som bevittnat våld idag inte är brottsoffer fullt ut juridiskt sätt och att detta gör det svårare för de att hjälpa dessa barn. Alla våra informanter var eniga om att det idag behövs en lagändring som gör det lättare för de att höra dessa barn och att barnen på så sätt kan få den hjälp och det stöd de behöver. Däremot var de eniga om att alla barn de träffar värderas och bemöts på samma sätt.

    Abstract [en]

    Background: Touching exists in different forms and can be experienced through the skin. By touching oxytocin is released like a relaxing hormone which counteracts anxiety and depression. Therapeutic contact and healing touch are methods nurses’ uses to give positive feeling of serenity, warmth and wellbeing for the patients. Touching can be a method for communication and happiness when word is not enough. By touching can nurses notice if a patient is, for example, anxious. The hands of the nurse are important caring of the patient such as adequate touching calms and sooths anxiety and pain.

    Aim:  To describe the significance to give touch in nursing.

    Method: A literary summary of scientific articles published between 1995 until 2012, where seven of them have qualitative approach and three have quantitative and all of them found via the databases CINAHL and MEDLINE.

    Result: Nurses were describing how touching had positive significance for the patients. The nurses could by touching reduce the patient’s pain, anxiety and worries. Different sorts of touching was mentioned like massage. A general theme was touching by non-verbal communication which gave a sense of security to the patients. The nurses’ attitude to touch became a problem since the nurses only touched the patients when necessary, for example when caring by hygiene without respecting the patients’ integrity.

    Discussion: In the result discussion it was described how the limited knowledge by the nurses of touching. By training (and education) they could carry though touching with help of different types of touching, massage, tactile touching, therapeutic touch etc. It relieved the state of pain, anxiety, and it also created security, closeness, better sleep and well-being of the patient. The touch adapted to the patients’ needs was described as positive and meaningful. To build an interpersonal relationship was essential and fundamental before the nurse approached the patient.

    Abstract [en]

    Background: In the natural aging process, changes occur in the body that affect the elderly person's daily life. Aging often leads to diseases that require treatment in form of drugs. The term polypharmacy describes the use of multiple drugs. Medications are the most common form of treatment and may contribute to both positive and negative effects on elderly’s health.

    Aim: The aim was to illuminate nursing problems in elderly regarding polypharmacy.

    Method: A literature review was performed based on 11 quantitative scientific articles. Databases used were Cinahl, PubMed and MedLine. Among the keywords used were polypharmacy, aged, risk factors and nursing. The theoretical framework was nurse Carnevali’s model (1996) dealing with nursing diagnosis.

    Results: From the selected articles’ results revealed four main categories were: 1) fall and risk of falling, which relate to how the elderly’s risk of falling increases with intake of many medications and its consequences, 2) non- adherence highlights the reasons why prescriptions are not followed neither by patients nor healthcare professionals. 3) Nutritional status and malnutrition concerns how polypharmacy can affect the elderly’s nutritional status and as a consequence lead to for example malnutrition, and 4) functional decline and the elderly’s (dis)ability to function in daily life is the last major category that could be affected by polypharmacy.

    Discussion: A difficulty that emerges from the literature is that the definition of polypharmacy is not identified. Despite this, it appears the nursing problems related to drugs can also correlate with multimorbidity which is common in the natural aging process.

    Abstract [en]

    Background: Globally the use of complementary and alternative medicine (CAM) has dramatically increased in the last few decades. In India CAM is used both as medical treatment and health care methods enhancing wellness.

    Aim: To explore nurse’s experiences of complementary alternative medical care as care methods in clinical settings in Karnataka, India.

    Methods: A qualitative interview study with semi-structured interviews was carried out. Four individual interviews were conducted with nurses working in two clinical settings in Karnataka, India. The data was analysed using a content analysis. 

    Results: Two main themes emerged from the content analysis: Caring as a challenge and Dealing with challenges.

    Discussions: The result is discussed using Travelbee’s’ theory on interactions.

    Abstract [en]

    The aim of the current paper is to investigate the precedent riders to 3 § LVU legislation that emerged in 2010 and how it has been implemented since in Stockholm county.  Moreover we aim to shed light on the type of treatment these young people should receive in order to thrive. To accomplish this we have assumed three research questions to answer.  Our chosen approach is based on a true dogmatic method and through a thorough qualitative literature review. In order to understand and interpret the system appropriately in implementation of the precedent legislation we have applied the new institutionalism organization theory to our analysis and conclusion.

         We have based our study on 15 LVU legal cases from 2013, involving young people with ASD or ADHD diagnosis. By interpreting and analyzing these court cases we are able to answer our first research question of how the precedent legislation rider is being implemented in Stockholm County. The result show that in two of fifteen cases the court took the precedent rider legislation into account and rejected applications of compulsory care.  In another two cases even though court has acknowledged and referred to the rider legislation, they have still judged in accordance to the 3 § LVU legislation prior to 2010 rider legislation. Finally in the last eleven cases the court has neither mentioned nor taken into account the precedent rider legislation.

         The other two research questions are established and answered through a thorough look at available literature and a review of previous research.  The result demonstrates the importance of early intervention and young people’s voluntarily participation in the support programs that county offers. The studies recommend that support and care should be provided initially in young person’s own home. If unsuccessful a family placement or HVB placement should be considered as a second option.

         According to the psychologist HejlskovElvén (2010) and SKL children and young people diagnosed with ASD and ADHD should be treated in accordance of a tailored treatment approach.  Involuntarily care as a punishment is not recommended as these young people are not in control of their behaviour or actions.  Presently involuntarily care is based on the youth’s socially destructive behaviour:  behaviour that deviates from the norms of society and that at a neuropsychiatric disability shall not be considered as such.  To ensure best care for young people diagnosed with ASD and ADHD cooperation between the various actors are vital as it helps to steer clear of the risk of them falling through the cracks as it unfortunately is often the case.

    Abstract [en]

    Background: There is no singular reason why a person ends up in substance abuse. Substance abuse poses health risks, which means that they will come in contact with health services. In the society the substance abusers are stigmatized which may affect the nurses’ behavior towards them.

    Aim: Highlighting the aspects that contribute to the nurse’s attitudes towards patients with a substance abuse problem and how the attitudes affect the health care.

    Method: The literature review is based on twelve scientific studies. The studies were analyzed toillustrate the similarities and differences. These were discussed and when the authors agreed it was written into the literature review's results.

    Results: The results showed that the nurse’s previous experiences had a big impact on how the nurses saw patients and how their attitudes influenced their expression. Previous experience contributed to fear and insecurity among many of the nurses. The professional competence turned out to have an impact because the nurse’s lack of knowledge of how an addiction affects prevented them to see the patient as a human being. Nurses with a high level of caring awareness managed this better.

    Discussion: The discussion includes Ericsson’s theory of suffering and the metaparadigm caring. Nurses’ attitudes affect the care relationship and that negative attitudes create an unnecessary suffering related to care and illness because there is a risk of information being overlooked. Social stigma affects how the nurses perceive the addict as a patient. A paternalistic approach may be necessary to increase the patients’ chances to have a future autonomy.

    Sammanfattning/abstract

    Bakgrund: En transsexuell person upplever sig tillhöra det motsatta könet och vill oftast korrigera sin kropp genom hormonbehandling och kirurgi så att den stämmer överens med det kön som personen identifierar sig med. Enligt den vårdvetenskapliga livsvärldsteorin är en människas kropp inte bara säte för hennes upplevelser utan också för hennes identitet. Genom kroppens sinnen skapar man en bild av sig själv och sin omvärld. Detta medför att om kroppen förändras, förändras även identiteten och tillgången till världen. Kunskapen om transsexuella personer hos personal inom hälso- och sjukvården är mycket bristfällig. Transsexuella personer utsätts inte sällan för diskriminering, intolerans och utanförskap.

    Syfte: Syftet med vår uppsats är att belysa transsexuella personers upplevelser av identitet och förkroppsligande.

    Metod: En litteraturstudie baserad på åtta vetenskapliga kvalitativa artiklar utfördes. Innehållsanalysen av datan gjordes enligt Granheim och Lundman.

    Resultat: Fyra kategorier framkom ur analysprocessen, dessa är: Den kroppsliga transitionen, som behandlar de transsexuella personernas upplevelser av kroppens förändring i relation till en könsbytesprocess. Inre och yttre identitet, som handlar om upplevelser av överensstämmelse mellan den kropp man fötts i och den inre identiteten. Familjerelationer som behandlar olika positiva och negativa aspekter av familj och vänners reaktioner på den transexuella familjemedlemmen. Den sista kategorin Att vara transsexuell i samhället och det offentliga rummet handlar om hur transsexuella personer upplever det bemötande de får och hur samhället ser på dem.

    Diskussion: Det som diskuteras i resultatdiskussionen är betydelsen av att se på kroppen som ”levd” för att förstå den kroppsliga transitionen, inkongruensen som uppstår mellan den inre känslan av könsidentitet och det yttre biologiska könet och att vara transsexuell i ett heteronormativt vårdsammanhang och samhälle. Detta diskuteras utifrån livsvärldsteorin och teorin om den levda kroppen och lyfts ytterligare genom begreppet Othering. Även etiska förhållningssätt diskuteras i samband med frågan om vården och samhällets heteronormativitet.

    Abstract [en]

    The purpose of this study is to investigate the lived experiences and treatment of Muslim women in their encounters with the social services. The primal themes for this study was: How does Muslim women experience the treatment of their case in the social services in a situation of need? How can the encounter and experience with the social services affect the clients trust in the social services?  What is the ideal treatment of women with a Muslim identity by the social services?

    The aim of this study was to investigate and capture the respondents lived perceptions and experiences with the social services, and thus the qualitative interview methodology was implemented to answer the research questions and hypotheses. Seven interviews with Muslim women were conducted in various districts of Stockholm, a city in northern Sweden. Five of the respondents were of ethnic Swedish origin and two were of other ethic origin.

    The results show that the majority of the respondents experienced a negative response at least once when in contact with the social services. Respondents emphasize that the unsafe and insecure meeting with the social worker was due to a non-verbal body language which they experience as an indication of preconceived notions about Muslim women, which is particularly experienced and described by the respondents with the Islamic headscarf. The trust in the social services decline in connection with the negative reflection of the respondents.

    However, the lived experiences with the social worker and the social services have not all been of a negative nature. The suggested ideal is also taken from the respondents own lived experiences with the social services.  A pervasive and consistent ideal is that the respondents want to be listened to, understood and respected for who they are as well as empowered rather than suppressed.

    Abstract [en]

    Introduction: In ISTDP have psychodynamic diagnostics developed and is continuously used in the treatment. The diagnostics divides patients into different character structures. Frederickson (2013) makes a classification into these four categories low resistance, moderate resistance, high resistance and fragile. This study examines how patients assessed character structure correlates with the patient's self-assessment of their mental problems.

    Questions: Does therapist's assessment correlate, concerning the nature of the structures low resistance, moderate resistance, high resistance and fragile, with the patient's self-rated mental health problems with the evaluation form CORE-OM.

    Methods: 31 patients at three psychiatric clinics have completed the self-assessment form CORE-OM. Then they were interviewed by the psychologists to assess the character of the structure. The estimated structure was compared with the outcomes of the CORE-OM. The statistical analyzes were conducted with Spearman's rho value. Kruskal - Wallis rank test. Selected level of significance <0:05. Follow-up pairwise mail / hoc test (Man-Whitney U test with Bonferoni corrected p value).

    Results: The estimated character structure correlates with the patient's self-assessment of psychological disorders as measured by the CORE-OM. The differences are significant between character structures moderate resistance and fragile regard both the CORE-OM total and domain scores. Fragile character structure stands out significantly from the rest of the domain risk with higher degree of risk reported. Character Structure moderate resistance stands out significantly with reported lower degree of difficulty in subcategories close relationships and socially with the rest.

    Discussion: The study's results demonstrate that the assessment of the character structure based ISTDP is a reliable diagnostic method. The results support the long tradition of theory in the psychodynamic tradition of the link between the nature of the defense and the level of mental illness.

    Abstract [en]

    Background: Dementia is the fourth most common endemic disease in Sweden and the number of people diagnosed with dementia is estimated to increase. Dementia includes diseases that affect the brain with symptoms on memory function, mood and communication skills. Several people with dementia is cared for at home by family members. Being an informal caregiver is a large responsibility and affects the whole family.

    Aim: To describe relative’s experiences of caring for a family member with dementia.

    Method: A literature review based on the analysis of ten scientific articles with qualitative design.

    Results: The results showed both positive and negative experiences. Some relatives expressed the feeling of becoming stronger and more resilient as a result of caring for their loved one but there where feelings of anger and frustration as well. The results showed that the relatives where in need of social and formal support but often had a hard time asking for help due to the feeling of being a burden for others and the society. Many experienced that their family member’s identity changed with the disease, which caused a change in the relationship as well. Some expressed role changes within the family and the lack of social and emotional closeness, while others expressed a feeling of becoming closer to each other.

    Discussions: It became clear that there was a complexity in relative’s attitude to social and formal support and nurses in contact with informal caregivers should be aware of this. Knowledge about informal caregiver’s perceived burden and what they perceive as their strength is an important key part for nurses in their work with dementia patients and their relatives. Nurses need to be aware of the occurrence of feelings of anger and frustration in relatives to be able to spot these and make sure the patient welfare is not compromised. Due to differences in men and women’s experiences it could be useful for the nurse to have a gender aspect in their meeting and caring for informal caregivers. The results were discussed from the perspective of Dorothea Orems self-care theory.

    Abstract [en]

    Background: Type 2 diabetes is a chronic disease that vigorously increases worldwide. The disease can affect the entire everyday life which usually results in major lifestyle change. The treatment consists largely of self care which the patient itself has great responsibility for. The nurses have an important role in supporting, guiding and teaching the patients. Since every patient is unique the nurse would benefit from an increased knowledge about the patients experiences of what promotes and prevents self care in type 2 diabetes.

     

    Aim: The aim was, from a patient perspective, describe the experience of what promotes and prevents a self care in type 2 diabetes.

     

    Methods: A literature review based on nine scientific articles gathered from the databases CINAHL plus with full text, PubMed and Medline. The articles were analyzed and common denominators were identified and themes created.

     

    Results: The patients promoting and preventing experiences were presented in two main themes with associated subthemes. To live with type 2 diabetes enlightes the patients experiences of personal perception, integration, driving force and interact with the surroundings. To meet healthcare illustrated the patients experiences of the caregivers supporting and informative role.

     

    Discussions: This study's results are discussed, developed and strengthened with other scientific studies, Dorothea E. Orem´s theory of self care, this study´s background and the author´s own reflections. It is revealed that no patient is the other alike and they should be treated as the unique individuals they are to enable the self care to be integrated in their life.
