    Abstract [sv]

    Två tidsmässigt parallella skeenden kan iakttas inom det sociala arbetet. Å ena sidan pågår en process mot att höja arbetets kvalitet. Helt centralt är här att den sociala praktiken ska styras genom enhetliga riktlinjer och bygga på vetenskaplig, forskningsbaserad kunskap. En form av standardisering som initialt har riktats mot den kommunala socialtjänsten, även om arbetet i ideell och privat sektor också har berörts. Å den andra sidan kon­trakterar stat, kommuner och landsting ut alltmer socialt inriktad verksamhet på alternativa utförare i idébaserade organisationer och på företag. En förhoppning finns om att dessa verksamheter ska bidra med ökad metodologisk och värdemässig mångfald. Inte sällan tillskrivs just de idébaserade organisationerna en särart som i sig sägs kunna skapa mervärden och som i en förlängning skulle utgöra grunden till en efterfrågad pluralism. Hur förhåller man sig i en socialt inriktad idébaserad organisation när de här två parallella utvecklingslinjerna möts, när förväntningar om särart och pluralism korsas av krav på standardisering och en evidensbaserad praktik?

     I rapporten Utrymme för flexibilitet får vi följa hur verksamma inom Credo, familjehemsvärdar som arbetar för Credo samt uppdragsgivare som anlitar Credo, utifrån sina tre specifika perspektiv beskriver Credos verksamhet. Hur tar sig värdegrunden uttryck i det praktiska arbetet, beskriver man någon form av särart i Credos verksamhet och vad för den i så fall med sig av eventuella mervärden? Hur beskriver man de metoder som Credo arbetar med och hur förhåller man sig till frågor om evidensbasering och kvalitet?

    Abstract [en]

    The Co-ordination Action on Human Rights Violations (CAHRV, 2004–2006) addresses human rights violations in the context of interpersonal relationships. CAHRV is a broad-based collaboration between research institutions, policy networks, and individual researchers and funded through the European Commission's 6th Framework Programme. Major goals of the action are to integrate parallel research discourses on violence, unify the theoretical and empirical basis for policy, stimulate new interdisciplinary national and trans-national research, and support practitioners, policy makers, and scientists by facilitating the dissemination of knowledge and expertise1 . CAHRV focuses on interpersonal violence, centred conceptually and strategically within a human rights discourse, with the aims of integrating relevant strands of research. Part of this larger project is a focus on interventions in gender-based human rights violations. The aims in this report are to develop a systematic overview of research on the successes and failures of policy and professional practice of statutory and NGO agents in addressing interpersonal violence and to explore the role of civic participation in addressing gendered violence. The companion report focuses on the intersections between criminal, civil and family law in response to gender-based violence.

    Abstract [en]

    Background: When a patient receives a diagnosis or contracts a disease, treatment can be focused on curation or palliation. Care with a curative orientation aims to cure the disease, whilst palliative care aims to relieve through supportive measures. Undergoing a transition affects all human dimensions. One of the nurse's tasks is to support the patient through transitions, doing so requires a trusting relationship between patient and nurse.

    Aim: The aim was to highlight patients´ experiences through the transition from curative to palliative care.

    Method: The method used was a literature review that aims to create an overview of the chosen field of knowledge. The authors created the result using ten original articles from the databases: Cinahl Complete, PubMed and Medline.

    Results: The result of the literature overview shows four main themes and four subthemes. The first theme "The nursing relationship" has two subthemes "The need to get information" and "The patients´ experience of the support from the nurse" describes patients´ experience of getting information and introduction to palliative care and the experience of the support from the nurse. The second theme "From independent to dependent" portrays patients' experience of changing roles and a fear of becoming a burden to someone else with one subtheme "The fear of being a burden to someone else". The third theme "Thoughts on the Future" is about patients' experiences and thoughts about the future during the transition from curative to palliative care. The fourth theme "Experiences of adapting to their new situation," describes patients' experiences of trying to adapt to their new situation.

    Discussion: Based on Meleis transition theory the results were discussed and related to the patient's experience of the transition from curative to palliative care. Getting the information has been important for the patients in this transition, which also confirms by the transition theory. The authors highlighted the problem of role changes and the experience of hope in the discussion.

    Abstract [en]

    Background: Diabetes type 1 is a disease that causes big changes in a person's life. The changes include various transitions, for an example transition from healthy to ill but also the transition from pediatric care to adult care. There are organizational differences between the pediatric care and adult care. Nurses have different roles in these two different treatment modalities that leads to the support being different too.

    Aim: The aim of this study was to describe patient’s experiences of transition from pediatric care to adult care of diabetes type 1.

    Method: A literature overview was used to structure and give the reader an overview of the selected area and to answer the purpose. Ten original scientific articles were used to compiled research that fulfill the purpose of this study

    Results: The result showed the patients personal views about the challenges during the transition. Patients had different feelings about the transition from pediatric care to adult care. The patients felt that improvements should be made in the organization level due to the bad structure during the transition. According to patients improvements should also be made with communication.

    Discussions: Nurses are in need of more knowledge about patients experiences of transition in order to improve their own role as a nurse. The patients feel stress and strain because of the transition from pediatric care to adult care. The patients felt stress also because there was no guidance from the health system under the transition period. The pediatric care and the adult care are in need of a structure to be able to decrease the unrest for patients.

    Background: Living with a psychiatric diagnosis may entail numerous challenges for the individual. People diagnosed with mental illness can daily experience a vulnerability to prejudice, not only in their everyday life in society but also within the healthcare context. Comorbidity is a substantial problem among a considerable part of the population with psychiatric diagnosis. Healthcare staff has described experiences of fear and uncertainty when dealing with people with psychiatric diagnosis, they enlightened a lack of knowledge when it comes to properly responding to the needs of people with a psychiatric diagnosis.

    Aim: The aim of this study was to investigate the experiences people with mental illness describe in contact with somatic healthcare.

    Method: The authors carried out a literature review. Through searches in databases such as Cinahl Complete and PubMed they have found articles that were analyzed and divided into themes.

    Result: The result of the literature review identified one main theme: Psychiatric patients’ experiences of somatic care and two subthemes: Healthcare staffs’ treatment and the treatments effect on patient participation.

    Discussion: The results found in the literature review were discussed in relation to chosen commitments from the tidal model. Reasoning arguments concerning treatment and ways to promote patient participation were held and discussed.

    Abstract [en]

    Statistics and fathers´experiences from custody battles imply that mothers have more benefits when the court is ruling for sole custody. In our study we examine if norms and conceptions regarding parenting roles and gender roles might affect the outcome in these custody battles. Our empirical material is based on court documents from Stockholm District Court in 2018. We have created statistics from 121 custody battle court orders, and performed a qualitative study based on 18 custody battle court orders. We use Norman Fairclough´s critical discourse analysis as our theory and method, and as a complement we use Parenting practices divided in four dimensions by Jane Lewis and Elaine Welsh a theory, as well as research about gender roles av Ylva Elvin-Nowak.

    Our research show a high number of cases with violence towards the mother, where the child has been witness to the violence. Some court cases concern parents with existing drug abuse or mental illness. All of these cases effect the outcome of the custody battle, with disadvantage to the violent parent or the parent with drug abuse/mental illness. Dominating themes and discourses in the results are Risk analysis, The child´s current living arrangement, Gendered Care Ability, Strategies and A Mother´s sense of right to her child is more apparent than a Father´s sense of right to his child. The results show that the outcome of a sole custody battle is primarily based on the child´s current living arrangement. We can show that some care abilities have different implications for mothers and fathers, and that mothers can exclude the father from the family as a strategy and benefit from it. Our study depicts a more nuanced image of how fathers are judged not only in the absence of “feminine” qualities.

    Assessment of patients' needs and resources as a basis in supervision for individualised nursing care in nursing home wards: evaluation of an intervention study2000Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    The study in this thesis was conducted as an intervention project in three nursing home wards in Sweden, in a big city as well as in rural areas, one year in each ward. The intervention consisted of supervision for individualised and documented nursing care with a multidimensional assessment instrument, the Resident Assessment Instrument (RAI/MDS), used as a basis. The main aim was to evaluate the intervention in the wards.

    Study I aimed at reviewing nursing documentation in the wards before and after the intervention. Quantitative content analysis was used to review the total number of nursing documentation (n = 58 + 57). The greatest change seen after intervention was the writing of care plans for the individual patients. Daily notes increased in length and number but reflected mostly occasional situations, as before. Computer-triggered Resident Assessment Protocol (RAP) items, obtained from the RAI/MDS assessments and regarded as a means of quality assurance, was used to compare items in the nursing care plans. The aim of study II was to compare patients' life stories and current situations as told by carers (n = 30 + 30). Qualitative content analysis was used to disclose changes and to enable descriptions of patterns in the text. After the intervention the patient was more seen as a unique person with resources and abilities despite limitations, and the nurses' awareness of their own professional approach increased. In study III a phenomenological-hermeneutic approach was used to illuminate nurses' video recorded interactions in morning care sessions involving patients, in one ward, suffering from severe dementia. In order to disclose any changes in the nurses' interactions the video recordings were conducted before, during and after the intervention (n = 24). The meaning in the interactions between the nurse and the patient were communicated through the balancing in actions and reactions, verbal- as well as non- verbal, and was about confirmation and the lack of it. This could fluctuate at times and be present within the whole complex of nursing care situations. The intervention contributed to developing a relationship of intersubjectivity and thus a greater emphasis on confirming nursing care. In study IV stimulated recall interviews were used in connection with the video recordings in order to illuminate the significance of nurses' reflections on their interactions with the patients (n = 12). Another aim was to find out if any changes in their reflections took place across the intervention. A phenomenological-hermeneutic approach revealed that nurses improved their ability to verbalise their reflections and to be aware of their own influence on care quality. The meaning in nurses' reflections was about efforts to maintain a sense of dignity for the patients as well as for themselves. In study V the Cognitive Performance Scale (CPS) included in the RAI/MDS was used to demonstrate the cognitive performance in elderly persons on different levels of care (n = 1276). Cognitive impairments in different stages were found for all levels of care investigated. In nurses' views (n = 50) of using the RAI/MDS (V) a majority thought that the instrument could contribute to improved care quality, even if some of them were hesitant. As the instrument made nurses more aware of the need for information, they thought that information as well as knowledge about the patient increased. They stated in their comments that the assessment covered all areas, and that they sought information they had not paid attention to or bothered about before the intervention, and further, that more of the patients' capacity and interests were considered. The importance of continuing education and supervision was confirmed by the study. It seemed as if the supervision and the use of a comprehensive and detailed assessment tool contributed to increased knowledge about the patients and to efforts to see them as real persons even behind a dementia surface.

    Stimulated recall interviews were used in connection with carers' video-recorded interactions with patients suffering from severe dementia before, during and after a 1-year intervention involving supervision for individualized nursing care. The aim was to illuminate carers' reflections on their everyday life with the patients, and to find out if any changes took place across the intervention. A phenomenological-hermeneutic approach was used in the analysis, which revealed that carers' reflections were focused on the carers themselves, on the patients, on context and on the work itself in the shared everyday life. After repeated stimulated recall interviews, together with supervision every month, an improvement in carers' ability to verbalize their reflections and an awareness and knowledge about their own influence on care quality were seen. The interdependence between carers and patients made it necessary for the carers to cope with many complicated here-and-now situations, and in their reflections the carers kept coming back to their efforts to maintain a sense of dignity for the patients as well as for themselves. Reflection through stimulated recall seems to be an important tool for carers in dementia care to facilitate understanding and to help them learn through lived experience, thus developing their skills in nursing care.

The aim of the study was to compare patients' life stories and current situations as told by carers before and after 1 year of supervision, in which the Resident Assessment Instrument was used as a basis for individualized nursing care. Qualitative content analysis was used to disclose changes and to enable descriptions of patterns. After the intervention, two overall perspectives emerged from the analysis: the patient as a unique person with resources and abilities, despite limitations, and the carers' awareness of their own professional approach. It seemed as if the supervision and the use of a comprehensive and detailed assessment tool contributed to increased knowledge about the patients and to efforts to see them as real persons behind the dementia surface.

    Using standardized assessment instruments may help staff identify needs, problems and resources which could be a basis for nursing care, and facilitate and improve the quality of documentation. The Resident Assessment Instrument/Minimum Data Set (RAI/MDS) especially developed for the care of elderly people, was used as a basis for individualized and documented nursing care. This study was carried out to compare nursing documentation in three nursing home wards in Sweden, before and after a one-year period of supervised intervention. The review of documentation focused on structure and content in both nursing care plans and daily notes. The greatest change seen after intervention was the writing of care plans for the individual patients. Daily notes increased both in total and within parts of the nursing process used, but reflected mostly temporary situations. Even though the documentation of nursing care increased the most, it was the theme medical treatment which was the most extensive overall. A difference was seen between computer-triggered Resident Assessment Protocol (RAP) items, obtained from the RAI/MDS assessments, and items in the nursing care plans; the former could be regarded as a means of quality assurance and of making staff aware of the need for further discussions. The RAI/MDS instrument seems to be a useful tool for the dynamic process in nursing care delivered and as a basis for documentation. The documentation should communicate a patient's situation and progress, and if staff are to be able to use it in their everyday nursing care activity, it must be well-structured and freely available. The importance of continuing education and supervision in nursing documentation for development of a reliable source of information was confirmed by the present study.

    Multidimensional functional assessment is the basis of individualized care. It is especially important in the care of elderly, with the complexity of symptomatology and often with cognitive impairment present. An assessment instrument for elderly persons, used in this study, is the Resident Assessment Instrument/Minimum Data Set (RAI/MDS) and its incorporated MDS Cognitive Performance Scale (CPS). The purposes of the study were to demonstrate the cognitive performance in elderly persons in different levels of care by using the CPS and to elicit the views of staff on use of the RAI/MDS. Cognitive impairment was found in 1276 elderly persons in six levels of care studied, an important factor to consider when organizing care of elderly. An intervention study was carried out for 1 year in three nursing home wards, with training and supervision in implementation of the RAI/MDS including individualized and documented care. Part of a questionnaire was used to evaluate staff (n = 50) views on using the instrument. A majority of the staff thought that the RAI/MDS could contribute to the improvement of quality of care, documentation in nursing records, and in co-operation and engagement. Further research is necessary to elicit more knowledge on the usefulness and benefits of the instrument.

    1. A phenomenological-hermeneutic approach was used to illuminate carers' video-recorded interactions in connection with supervision for individualized nursing care. 2. In order to disclose any changes in the carers' interactions with patients suffering from severe dementia the video recordings were conducted before, during and after the intervention. 3. The content of the videos was transcribed as a text, mainly verbal communication. Due to the rich data the videos and text were kept together as a whole in every step of the analysis. 4. After an initial naïve understanding, different subthemes emerged in the structural analyses: promoting competence, struggling for co-operation, deep communication for communion, showing respect for the unique person, skills in balancing power, distance in a negative point of view, and fragmentary nursing situations. 5. The overall theme was 'Carers' balancing in their interactions, verbal as well as non-verbal, to promote a sense of mutual togetherness with the patient'. 6. The supervision intervention contributed to an improvement in carers' skills in balancing in their interactions. In the caring process carers' and patients' shared experiences and, due to patients' disabilities, interactions depended mainly on carers' qualities and capabilities for this confirming nursing care.

    Abstract [en]

    BACKGROUND: An intervention project was conducted in three nursing home wards in Sweden. Most patients had severe dementia. The intervention consisted of supervision for individualized and documented nursing care, based on multidimensional assessment. AIM: To illuminate changes in carers' approach after the intervention. METHODS: Several data collections were conducted across the intervention and consisted of nursing documentation, patient life stories as told by carers, video recorded interactions, stimulated recall interviews and a questionnaire. Both quantitative and qualitative methods were used in the analyses. FINDINGS: The findings from the different methods mirrored each other and added to the credibility of the intervention. Communicated knowledge about patients improved in nursing documentation and also as told by carers. Carers were differently skilled in managing the complexity of nursing care situations before as well as after the intervention, but the intervention contributed to developing carers in 'confirming nursing care'. They also improved in their ability to verbalize reflections about their everyday life with patients with dementia. CONCLUSION: Supervision made it possible for carers to share their lived experiences about their day-to-day life with patients, which could promote personal and professional development and thus improve care quality. It also appeared that a detailed assessment tool used as part of the nursing process contributed to seeing a patient as a real person behind a dementia surface.

    Abstract [en]

    Background: Inflammatory bowel disease (IBD) encompasses ulcerative colitis and Crohn's disease. They are chronic and occur in relapses when the mucosal becomes inflamed and ulcerated, causing complex symptoms such as acute diarrhoea, rectal bleeding and abdominal pain. The treatment aims to reduce symptoms and prevent the occurrence of relapses. As these diseases affect the daily life it also affects the individual health-related quality of life, which measures the physical, psychological and social aspects of a health problem.

    Aim: The purpose was to describe the experience of health-related quality of life in people with inflammatory bowel disease.

    Method: A literature review has been done where twelve original articles have formed the basis of the results, this creates an overview of existing research. They have been read, summarized and analysed where the similarities and differences were found and themes were formed.

    Results: The four main themes of the result is Physical function, Psychological function, Social function and Rebuilding their quality of life. It emerged in the results that physical symptoms and disease activity are those which essentially controls the psychological well-being as well as social constraints. This was shown in studies to have a negative impact on the health-related quality of life. But overtime the disease became a normal part of one's life and gave a better quality of life experience.

    Discussions: The method approach, benefits and weaknesses will be discussed and analysed. When discussing the results of the current study mainly knowledge, education, and time perspective will be addressed and analysed with Orem’s self-care theory.

    Abstract [en]

    Background:The new Swedish law for patients from 2015 strengthens the position patients have in relation to health care providers. Person-centred care is something described in the competence description for specialist nurses in psychiatric care, in guidelines of care for persons who have psychosis and research about person-centred care shows that it can improve quality of care. Studies have shown that patients cared for in psychiatric care wish to be more involved in care and to be seen and respected as individuals. Research, however, is inadequate about the experience specialist nurses have of person-centred care in their clinical practice.

    Aim: To describe experiences that specialist nurses have of person-centred care in work with people with psychosis in psychiatric outpatient care.

    Method: The study was carried out with a qualitative design and the data was collected with semi-structured interviews with five specialist nurses in psychiatric outpatient care. Qualitative content analysis was used to analyze the data.

    Results: The data generated three main categories and 14 subcategories. The main categories were named: To shape a person-centred care, the importance of the caring relationship and to give space to the person cared for.

    Discussions: The result of the study was discussed in relation to The Tidal model, which is a theory for psychiatric nursing funded by Barker. The result was also discussed in relation to research, guidelines of care, competence description for specialist nurses and law.
