    Abstract [sv]

    Bakgrund: Begreppet hemlöshet innefattar flera situationer. Det är svårt att veta hur många hemlösa personer som befinner sig i Sverige då hemlöshet även innefattar de som lever utan uppehållstillstånd, papperslösa och asylsökande. Hemlösa personer löper en högre risk av att drabbas av både fysisk och psykisk ohälsa och dåliga erfarenheter av vården kombinerat med en begränsad ekonomi och oklarheter vart i vården de ska vända sig kan göra att hemlösa inte uppsöker vården. Sedan 2013 har både asylsökande och papperslösa rätt till vård som inte kan anstå och enligt hälso-sjukvårds lagen ska vården ges med respekt på lika villkor. Trots detta sker det att papperlösa och asylsökande nekas vård.

    Syfte: Att beskriva sjukvårdspersonals upplevelse av att vårda patienter som lever i hemlöshet.

    Metod: Litteraturöversikt med nio vetenskapliga artiklar som valdes ut från databasen Cinahl Complete, Swepub, Pubmed, PsycINFO och Academic Search Complete. Åtta var av kvalitativ karaktär och en använde kvantitativ metod. Dessa analyserades och sammanställdes till tre huvudkategorier och fem underkategorier.

    Resultat: Litteraturöversikten innefattar följande kategorier: Vårdpersonalens känslor inför hemlösa patienter, Utmaningar av att vårda hemlösa patienter, Hinder i vårdandet, Hinder vid kommunikation och dokumentation, Hinder och utmaningar inför hantering av komplexa vårdbehov, Förhållningsätt i mötet med hemlösa patienter, Ett öppet sinne och en vilja att vårda och Att skapa en vårdrelation med hemlösa patienter. Resultatet visar att vårdpersonalen kan ha både positiva och negativa känslor inför den hemlösa patienten och att det uppkommer hinder i vården av hemlösa. Vårdpersonalen förhållningsätt i vården av hemlösa och skapandet av en vårdrelation kan vara svårt men anses viktigt. 

    Diskussion: Här diskuteras delar av alla kategorier ifrån resultatet i relation till forskning och Katie Erikssons teori om att ansa, leka och lära och konsensusbegreppet vårdande.

    Abstract [en]

    We have analysed the CAG repeat in the Huntington disease (HD) gene in sperm and blood from 20 unrelated HD patients. Although the CAG repeat displayed significant mosaicism in sperm from all individuals, there were marked differences in the degree of repeat instability. Individuals who had either inherited or transmitted an expanded CAG repeat displayed the highest levels of repeat mosaicism, whereas individuals who had inherited or transmitted a contracted repeat had very limited CAG mosaicism in sperm. A strong association between intergenerational change in CAG allele size and the level of sperm repeat mosaicism was determined (P = 0.019). In contrast, neither blood CAG size nor repeat mosaicism in blood, were significantly associated with intergenerational CAG changes. These data suggest the presence of a cis-acting factor, separate from CAG size, that strongly influences the intergenerational behaviour of the CAG repeat. Additional studies are needed to determine whether analysis of CAG mosaicism in sperm is useful for assessing an individual's risk for transmitting large expansions or contractions to his offspring.

    Abstract [en]

    Juvenile Huntington disease (HD), characterised by onset of symptoms before the age of 20 with rigidity and intellectual decline, is associated with a predominance of affected fathers. In order to investigate the molecular basis for the observed parental effect, we have analysed the CAG trinucleotide repeat within the HD gene in 42 juvenile onset cases from 34 families. A highly significant correlation was found between the repeat length and age of onset (r = -0.86, p < 10(-7) and it was determined that the sex of transmitting parent was the major influence on CAG expansion leading to earlier onset. Neither the size of the parental upper allele, the age of parent at conception of juvenile onset child, nor the grandparental sex conferred a significant effect upon expansion. Affected sib pair analysis of CAG repeat length, however, revealed a high correlation (r = 0.91, p < 10(-7). Furthermore, analysis of nuclear and extended families showed a familial predisposition to juvenile onset disease. This study demonstrates that the sex of transmitting parent is the major influence on trinucleotide expansion and clinical features in juvenile Huntington disease.

    Abstract [en]

    Background: Mental illness is increasing in today’s society and is a serious public health problem. Negative attitudes and discrimination is common against people with mental illness. Some patients experience that they have been treated differently and that they are not involved in their own care. The nurse has a legal and ethical responsibility to maintain a good attitude and to conduct non-discriminatory care to avoid violations of patient rights. According to previous research nurses feel insecure and concerned regarding the care of patients with mental illness in somatic wards.

    Aim: To describe nurses' experience of caring for patients with mental illness in somatic wards.

    Method: A literature review based on ten scientific articles, of which six were qualitative and four were quantitative. Articles were sought out in the databases Cinahl Complete and PsycINFO.

    Results: The results are presented in four main themes; The nurse’s view on him-/herself, the nurse’s view on the patient, the nurse’s feelings and the nurse’s perception of the environment. Nurses experienced a lack of knowledge and had different attitudes and prejudices towards patients with mental illness. Feelings such as fear and anxiety, as well as environmental aspects were factors that aggravated the caring.

    Discussion: The discussion is divided in two parts, the method discussion and the result discussion. In the method discussion the method’s strengths and weaknesses are discussed. The result discussion discusses the results based on Barker’s nursing theory and its central term, “caring”.

    Abstract [en]

    Few studies shed light on what older people themselves consider a good or dignified death. This study is part of a larger project where interviews were used to follow 12 nursing home residents over time. The aim of the present study was to reach a deeper understanding of seven older people's thoughts, feelings and attitudes towards death. The participants were interviewed on four occasions across 18-24 months, and the interviews were analysed using a hermeneutic method. The findings revealed three main themes: 1. Zest for life, 2. Indifferent attitude, 3. Longing for death. The themes mirror the complexity of what ageing and nearing the end of life can mean. A strong desire among the participants was to be more involved in their own personal care and in everyday life at the nursing home. Person-oriented care based on the philosophy of palliative care, and including conversation as a natural component, might promote the older person's possibilities to obtain a good death.

    Abstract [en]

    Background: Epidemiological studies have shown that heart failure have increased all over the western world. An increase in the number of patients with heart failure can entail an increase in the demands on the healthcare system and healthcare personnel. It have shown to be necessary to educate all health care personnel, especially nurses and district nurses active in elderly care, in how to care for patients with chronic heart failure since when the patients deteriorate they are in great need of support.

    Aim: The aim of this literature review was to enlighten patients experiences of living with heart failure.

    Method: The conducted study was a literature review, according to Friberg (2012), containing eight scientific studies of qualitative design. Collection of data have been via PubMed and CINAHL Complete

     Results: The result identified three main themes with seven sub themes. The first main theme was: Experiences of changes in the life situation with the three following sub themes: Living with limitations, Fear of the future and Experiences of loneliness. The second main theme was: Experiences of symptoms and health care with the two following sub themes: Symptoms in daily life and Experiences of health care. The third main theme was: Strategies to facilitate life with heart failure with the two following sub themes: Adaptation and The importance of social support.

    Discussion: The result discussion enlightens the experiences of people with heart failure in relation to Roys adaptation model. People with heart failure have described different experiences of the illness, most of them involving adapting to the new life situation after the diagnosis.

    Abstract [en]

    Background: Being a relative to someone with mental disorder often causes a decreased mental health and can be both time-consuming and costly. Feelings of guilt and shame and stigmatization from the surroundings are common. Relatives’ wishes to participate in care but often get excluded. National guidelines and laws emphasize the importance of relatives’ participation; despite this relatives' often get perceived as intrusive. Little is known about relatives contact with psychiatric intensive care units.

    Aim: The aim of this study was to describe nurses’ experiences from meeting relatives at psychiatric intensive care units.

    Method: This study has a qualitative design with an inductive approach. Seven nurses at psychiatric intensive care units in Sweden were interviewed with semistructured method. The material was analyzed using a qualitative content analysis.

    Results: The result from the interviews developed into four categories: Organizational requirements to meet relatives, Different views on relatives, Challenges in the meeting with relatives, and To meet and communicate with relatives.

    Discussions: The result is being discussed in relation to previous research and Andershed's theory of Involvement in the light – Involvement in the dark. In the result aspects emerged that can lead to both participation in the light and in the darkness.

    Abstract [en]

    Background: Touch can be given planed, like the therapeutic and the tactile touch, many times with the purpose to ease symptoms. It can also be necessary, for example when helping a patient getting dressed, with the meaning of doing a task, even if it also can transmit feelings. There is also affective touch, transmitting feelings and sympathy. The nurse can look upon touch as a tool to reach the patient or to ease pain or anxiety. The experience of the patient is often missed when talking about touch, even though the touch should be individually conformed. Aim: The aim of this study is to describe patients’ experiences of touch. Method: A literature review inspired of Friberg (2006) consisting of nine qualitative articles, all found though the database of CINAHL. Result: The touch gave different feelings and experiences depending on what type of touch was received. The result is described in four themes; To feel cared for, to feel discomfort, to feel physically at ease and to get in contact with oneself. In these themes major parts showed on positive effects of touch, such as symptom ease, relaxation and wellbeing. The result also showed negative experiences of touch. It had to do with being touched by the opposite sex, or about a fear that the touch would hurt. There was also described a feeling of fear for what would happen when the touch ended. Discussion: A discussion is held about the caregivers’ way of being, about the discomfort touch can create and about touch as something positive. Previous research strengthens the result. There’s a discussion partly in relation to the description of the caring relation according to Katie Eriksson as well as to her theory about trim, play and learn, but also in relation to other literature in the subject.     Bakgrund: Mat och dryck är den biologiska faktorn som håller människan vid liv och redan vid födseln finns denna biologiska drivkraft i oss. När förmågan att tillföra nutrition via munnen sviktar finns bland annat alternativet peg, för att få tillräckligt med näring. Patienter som har peg går igenom en transition där de behöver adaptera till den nya situationen att få näring via sin sond. Det är av betydelse att sjuksköterskan har en stödjande funktion i de livsavgörande situationerna så att delaktighet och livskvalitet kan upprätthållas.

    Syfte: Syfte med litteraturöversikten var att belysa patienters upplevelse av att leva med Perkutan Endoskopisk Gastrostomi (peg).

    Metod: En litteraturöversikt baserat på Fribergs analysmetod har genomförts. Resultatet inkluderar tio vetenskapliga artiklar som sökts i databaserna CINAHL Complete och PubMed. Callista Roys adaptionsmodell har använts till litteraturöversiktens diskussionsdel.

    Resultat: Resultatanalysen genererade tre olika aspekter på hur patienter med peg upplever livet. Fysiska aspekter handlar om patienternas upplevelse av nutritionsstödet och komplikationerna. Sociala aspekter beskriver hur patienterna ser på måltidssituationerna och deras relationer. Psykologiska aspekter handlar om patienternas tillämpade strategier, hur de uppfattade sin delaktighet i beslutet samt om deras upplevelse av livskvalitet.

    Diskussion: Resultatet i litteraturöversikten har diskuterats utifrån Callista Roys adaptionsmodell. Den artificiella nutritionen inverkade på både patientens identitet och sociala kontext. De närstående hade också en betydelsefull roll för patienterna. Trots komplikationer och påverkan vid sociala sammanhang kände patienterna att pegen räddade deras liv.

    Abstract [en]

    Background:

    Moral stress is a term that was first defined by Jameton (1984) and is considered to occur when individuals are unable to act in accordance with what they consider to be right due to organizational or institutional barriers. The registered nurse is responsible for a large area of knowledge and work that needs to be characterized by an ethical approach and person-centered care. An internal conflict occurs when the nurse is prevented from working on the basis of these values, which causes feelings of moral stress. A greater knowledge and understanding of the problem area is necessary to achieve future improvements.

    Aim:

    The aim was to illustrate moral stress among the nurse.

    Method:

    To achieve this goal, a literature review has been conducted. Searches for relevant articles in accordance with the English translation of moral stress were performed. The databases Cinahl, PsycINFO and PubMed were used. Articles with qualitative and quantitative approaches have been used.

    Results:

    The analysis revealed three different main categories: underlying causes for moral stress, consequences of moral stress and strategies for managing moral stress.

    Discussion:

    In the result, moral stress is discussed among nurses based on Jean Watson's theory of human care and other relevant articles. The appearance of moral stress in nurses is discussed from different perspectives. Differences in the environment and management's significant role regarding nurses' feelings of moral stress are discussed further. The consequences of moral stress in nurses are discussed with emphasis on patient safety and the encounter of patients. Awareness about moral stress in nurses needs to be greater. A focus on moral stress in nurses should be emphasized during nursing education.

    Abstract [en]

    Despite the fact of increased levels of alcohol dependency in women and gender differences in how the disease affects men and women, the research on alcohol dependency continues to have a dominating perspective on men. The meaning of the phenomenon of caring in formal care for women with alcohol dependency is not well known. Thus, formal caregivers may find it problematic to know what is caring for women with alcohol dependency. The aim of the study was to illuminate the meaning of caring in formal care for women with alcohol dependency, as narrated by the women. The study was performed using a phenomenological-hermeneutic method. Data were collected in ten in-depth interviews with alcohol-dependent women. The themes presented are availability, being a patient and being a learner. The findings reveal that the women with alcohol dependency receiving a mandate from formal care, experience the relation between them and their caregiver as a mutual transformation. Within the mutual transformation, the participants experienced being respected as a responsible human being which renders possibilities for the women with alcohol dependency to continue in formal care even when the struggle against the disease became hard. Continual meetings with the caregiver allowed the women to gain structure in their daily life as well as allowing the women and their caregivers to develop mutual transformation, which both relieved the women's suffering and increased their experience of being involved in the care process.

    Abstract [sv]

    Bakgrund: Långvarig smärta är en subjektiv upplevelse som förkommer hos mellan 40 till 50 procent av den svenska befolkningen. Den kroppsliga förändringen som smärtan innebär påverkar identiteten, kontakten med världen och med andra människor. Att hantera smärtan och livssituationen är en utmaning som olika människor hanterar på olika sätt. Bristen på objektiva tecken bidrar till att stigmatisering av patienter med långvarig smärta förekommer såväl i privata relationer som i vårdrelationer.

    Syfte: Syftet med denna litteraturstudie är att beskriva hur patienter med långvarig smärta upplever och hanterar sin livssituation.

    Metod: För denna litteraturöversikt har elva vetenskapliga, kvalitativa artiklar analyserats. Artiklarna har relevans för syftet. Efter genomläsning med fokus på respektive studies resultatdel identifierades nyckelfynd. Likheter och skillnader mellan studierna identifierades och dessa fick utgöra nya teman och kategorier i resultatet i denna litteraturöversikt.

    Resultat: Resultatet presenteras i form av tre teman; upplevelser av den förändrade kroppen, hantering av en förändrad livssituation och smärta som en subjektiv, osynlig upplevelse. Att leva med långvarig smärta innebär inte bara att upplevelsen av kroppen förändras utan även att tillgången till världen förändras. Resultatet visar olika sätt på vilka patienter upplever och förhåller sig till dessa förändringar. Då smärta är en subjektiv och osynlig upplevelse adderar detta ytterligare svårigheter i mötet med andra människor.

    Diskussion: Katie Erikssons ontologiska hälsomodell används som teoretisk utgångspunkt i diskussionen. Långvarig smärta hade förändrat livet för patienterna i vår studie. Att integrera denna förändring var en process som olika patienter handskades med på olika sätt. Långvarig smärta kan innebära ett mångdimensionellt lidande för patienter och att enbart fokusera på en kroppslig nivå är inte tillräckligt. Erikssons hälsomodell belyser även den själsliga och den andliga dimensionen. Det är avgörande att låta patienten berätta om sina upplevelser och sin situation för att identifiera de områden som är viktiga för patientens liv och hälsa.

    Background: Millions of people are yearly diagnosed with a cancer disease. The cells in the body can for different reasons mutate and become tumours. If the tumour is determined as bad it’s considered cancerous. The responsible physician takes a stand if the cancer disease is treatable or if the treatment should be palliative. The palliative care should be shaped from the terms set by the patient. The patient ought to receive care from healthcare staff, where they respect the individual and treat them in a kind manner, so called person centered care. The patient is to be encouraged to have the freedom to control their own life, the right to information, preserve their integrity and receive adequate alleviation of symptoms.  

    Aim: A literature review over how patients with a cancer disease experience the encounters with healthcare professionals in inpatient palliative wards.

    Method: The databases CINAHL Complete and MEDLINE with Full text were used for information retrieval. The retrieved information was audited and approved by both authors.

    Results: The results highlight patient experiences of the professional-patient relation. The study resulted in two themes. The first theme: Healthcare Professional’s approachment with two subtopics: Presence and commitment and trust and respect. The second theme: Communication and information, without subtopics.

    Discussion: The results were discussed on the basis of Rosemarie Rizzo Parse's theory after the subjects trust and mistrust were identified. Patient experienced that involvement, honesty and benevolence were important. The experiences were discussed based on Parse's theory about Human Becoming and other literature.

    Hälso- och sjukvårdens utveckling har lett fram till ett marknadsstyrt system. För att kunna jämföra olika vårdgivare och utveckla kvalitén har ett flertal kvalitetsindikatorer och uppföljningsområden införts. Fallrisk är ett av dessa uppföljningsområden. I Sverige är Downton Fall Risk Index förstahandsval som fallriskbedömningsinstrument. Hälso- och sjukvård ska utformas och genomföras i samråd med patienterna. Den traditionella rollfördelningen mellan patienterna och sjuksköterskorna håller på att förändras till att patienternas rätt till delaktighet uppmärksammas.

    Syftet med denna uppsats var att belysa sjuksköterskornas erfarenhet av och syn på patienternas delaktighet i fallriskbedömningen.

    Det här är en intervjustudie med kvalitativ ansats. Åtta sjuksköterskor som arbetar på ett akutsjukhus i mellersta Sverige och använder sig av Downton Fall Risk Index vid fallriskbedömningen har intervjuats. Kvalitativ innehållsanalys som inspirerats av Graneheim och Lundman har använts. Den teoretiska utgångspunkten är Peplaus syn på relationen mellan sjuksköterskorna och patienterna som en av de viktigaste grundstenarna inom omvårdnaden.

    Studien kom fram till följande fem teman: fallrisk är sjuksköterskornas bedömning, standardmall har ett eget liv, fallrisk - skrämselpropaganda eller verklighet, sjuksköterskornas svårigheter med att involvera patienterna och sjuksköterskornas syn på patienternas delaktighet.

    Det standardiserade arbetssättet uppmuntrar inte till utvecklingen av reflektionsförmågan och kliniska blicken bland nyutbildade sjuksköterskor därför behövs det en kompetensstege för sjuksköterskor. Sjuksköterskor hittar inte plats varken för sin egen eller för patienternas delaktighet i Downton Fall Risk Index. Frågan som kan ställas är om rätt instrument har valts för fallriskbedömningen i akutsjukvården.

    Nursing today faces numerous challenges. Societal changes lead to reorganization of health care, changing workloads with sicker patients in hospital and home care, and limited economic resources. The increasing and changing nature of knowledge needed for expert care provision challenges nurses to continually update their competencies. These are issues demanding proactive and dynamic changes in the way nurses conceive their mandates and practice. The aim of the action-research project presented here was to foster improved quality of care for patients with advanced cancer through collaborative endeavours integrating cancer nursing clinical practice, research and education in a knowledge exchange programme. The programme was based on input about caregiving needs from multi-professional staff caring for patients with advanced cancer in a variety of healthcare settings. Undergraduate baccalaureate nursing students were then engaged in literature studies to help address these needs. Results of the studies were communicated back to the involved clinicians in a variety of ways. In this paper, we discuss what we have experienced as opportunities and obstacles in conducting the project, based on our reflections and external evaluations. This is linked to a broader discussion of ways of integrating cancer nursing research, education and practice.

    BACKGROUND: The gap between nursing research and practice is readily acknowledged in literature, with a variety of strategies suggested for reducing this gap. It is necessary not only to address problems of research implementation in practice, but also to find strategies that strengthen the influence of practitioners on research agendas through more collaborative relationships in order to have an impact on care of patients. A multi-centre research project was therefore initiated by two universities and three health care facilities, aiming to improve quality of care for patients with advanced cancer through a knowledge-exchange programme between nurse researchers, practitioners and students.

    AIM: The aim of this article is to explore how clinical staff reason about care provision for patients with advanced cancer, through analysis of 20 focus group discussions conducted with staff in three different health care facilities in two Swedish cities. An initial analysis based on grounded theory was complemented with consideration of the interactive process in the focus group discussions, and carried out by a team consisting of senior nurse researchers, clinical experts and nursing instructors.

    FINDINGS: The findings of the focus group discussions emphasize the complexity of caregiving for patients with advanced cancer. The tension between caregiving ideals and limits imposed by the realities of caregiving in today's health system were striking. Practitioners discussed the organization of care, different constellations of relationships between patients, family members and professionals, and theoretical and experiential knowledge as equally important aspects in dealing with all concrete situations in daily practice. The importance of reflective practice, use of self and ethical reasoning also permeated the focus group discussions.

    CONCLUSIONS: These findings highlight an integrated need both to influence organizational structures and working relationships, along with increasing knowledge, if sustainable change is to be effected.

    Abstract [en]

    Background: Specialist nursing education in psychiatric care is based on basic nurse education. Specialist nurses will perform nursing at advanced level. They are designated as nursing experts to constantly reflect on their own actions as well as demonstrate a specific knowledge of their duties.

    Aim: The aim of the study was to describe specialized nurses` experiences of how their specialisteducation in psychiatric care influence their work in mental health inpatient setting.

    Method: Six qualitative interviews were conducted with specialist nurses from two psychiatric hospitals in Sweden. The interviews were semi-structured and were made with the support of an interview guide with an open primary main question and support questions. Qualitative design was chosen for this study and data were analyzed using qualitative content analysis inspired by Graneheim and Lundman

    Results: From the data analysis revealed five categories. These were: insight about the importance of cooperation with patients, relatives, colleagues and other care agents; starched in the role of leader; advanced understanding of psychiatric diseases, care and treatment; personal and professional progress; improved capability of patients` and employees` treatment. Specialist nurses expressed that the specialist nursing education is considered to have a positive influence on their work in psychiatric care.

    Discussions: The results are discussed in realtion to Benner, Tanner and Cheslas vision of nurse as an expert in nursing skills and knowledge and to previous literature. The study shows how strongly the specialized nurses feel the influence of education in their work duties. Some voices point out that specialist nursing education should be adapted to participants' needs.

    Abstract [en]

    Background: Adverse events are common in Swedish and foreign healthcare. These events overall contribute to several days of prolonged hospital stay and occupies a large number of hospital beds. It occupies resources and costs the society billions of Swedish crowns (SEK) every year. Healthcare professionals are responsible for the patients’ safety in the healthcare, which in Sweden is regulated by laws and regulations. These are designed to protect patients against adverse events and injuries in the healthcare and entail obligations for healthcare professionals to constantly work to develop the quality and safety for the patients in the healthcare. 

    Aim: The aim of this literature review was to describe patients´ experiences of being affected by an adverse event.

    Method: A literature review has been performed according to Friberg’s method. The databases CINAHL Complete and PubMed were used for searching articles for this literature review. Eleven original articles were found, which are used to form the results of this review.

    Results: With support in Katie Eriksson´s theory of caring, three main categories of patients experiences were found, these categories were suffering related to healthcare, suffering from illness, suffering of life. Each category had related subcategories where patients' experiences are described.

    Discussion: Main findings has been discussed on the basis of the background, other research and Katie Eriksson’s theory of caring. A lack of communication and information from the healthcare professionals after an adverse event was experienced. In cases when the communication and information perceived satisfactorily the suffer were reduced. Patients’ experiences show that healthcare professionals do not follow their responsibilities and the existing laws, which inhibits the quality and safety of the healthcare. In contrast to the negative experiences there were also positive experiences, which are related to good care before and after the adverse event.

    Abstract [en]

    OBJECTIVE: The mutation responsible for Huntington's disease is an elongated and unstable trinucleotide (CAG) repeat on the short arm of chromosome 4. Psychotic symptoms are more common in patients with Huntington's disease than in the general population. This study explored the relationship of psychosis in Huntington's disease patients with the number of CAG repeats and family history of psychosis.

    METHOD: Forty-four patients with Huntington's disease, 22 with and 22 without psychotic symptoms, were recruited from two university-affiliated medical genetics clinics in Seattle and Vancouver, B.C. Psychiatric assessments of the subjects were made through chart review, and diagnoses were validated by structured interviews in a subset of patients. The demographic and clinical characteristics of the psychotic and nonpsychotic patients were compared.

    RESULTS: The two groups did not differ in demographic and clinical characteristics, except that subjects with psychosis were significantly more likely than nonpsychotic subjects to have a first-degree relative with psychosis. In eight of nine families in which Huntington's disease probands with psychosis had a first-degree relative with psychosis, the relative's psychosis co-occurred with Huntington's disease. In the Huntington's disease probands with psychosis, the onset of psychosis correlated with the onset of the neurological symptoms of Huntington's disease, and the age at onset of psychosis was lower in probands with a higher number of CAG repeats.

    CONCLUSIONS: Patients with Huntington's disease and psychotic symptoms may have a familial predisposition to develop psychosis. This finding suggests that other genetic factors may influence susceptibility to a particular phenotype precipitated by CAG expansion in the Huntington's disease gene.

    Bakgrund År 1974 framförde svensk författningssamling abortlagen som gav kvinnan rättighet att genomgå en legal abort fram till graviditetsvecka 18. Abort innebär avbrytande av havandeskap som genomförs av två metoder, medicinsk och kirurgisk abort. Sjuksköterskan har en stor roll inom abortprocessen som innefattar tidsbokning, rådgivning, administrering av läkemedel, smärtlindring och efterkontroller.

    Syfte Syftet var att beskriva sjuksköterskors upplevelser och attityder av att vårda kvinnor som gör en planerad abort.

    Metod En litteraturstudie har gjorts för att kunna göra en sammanställning om de studier som finns inom ämnesområdet. De tillämpades elva artiklar varav nio av dessa var kvalitativa och två av dessa var kvantitativa. Artiklarna som valdes till studien kvalitetsgranskades enligt Forsberg och Wengströms (2008) beskrivningar och analyserades med hjälp av deras beskrivningar på hur man analyserar vetenskapliga artiklar. The middle range theory of caring har knutits an till studien.   

    Resultat Utifrån artiklar som granskats i denna litteraturstudie utvecklades fem teman fram: rätt eller fel att göra abort, otillräckligt stöd till sjuksköterskor, att hantera sina upplevelser och attityder - professionellt förhållningssätt, sjuksköterskors upplevelser och attityder av att uppnå rättvis vård och upplevelser om att uppnå en personcentrerad vård för kvinnor.

    Diskussion Utifrån studien observeras det att det finns olika upplevelser kring abort som har anknytning till personens kulturella, religiösa och personliga åsikter om det är rätt eller fel att göra abort, som gör abort till ett kontroversiellt ämne där sjuksköterskor ofta blir tvungna att bemöta många känsloladdade och påfrestande situationer. Därför blir det viktigt med mer stöd i form av information, stödsamtal, rådgivning och undervisning till sjuksköterskor för att kunna hantera arbetssituationen och mötet med patienten på ett bättre sätt.

    Abstract [en]

    Background: People are sexual beings from birth to death and to be able to express one’s sexuality is important for one´s health, well-being, quality of life, and is also part of the human rights. Notwithstanding, there are theories in society that older people do not have sexual desires or sexual feelings. In today’s media it is reflected that the elderly’s sexuality is either non-existent or unacceptable. The role of health personnel is to promote the well-being and quality of life of humans which also includes sexual health. This is why it is important to acquire more knowledge about health personnel’s attitudes to older people's sexuality.

    Aim: The aim was that through a literature review describe the nursing staff 's attitude towards older people 's sexuality.

    Method: A literature review was conducted with the help of eight scientific studies out of which three have a qualitative design, four a quantitative design and one uses a mixed method. Restrictions included 2000-2015 , Age 65 and up, peer rewied and English. Selected keywords were Aged sexuality, Aging, Attitudes, Health Personnel, Nurses, Nursing care , Nursing Homes, Older People Sexual Behavior , Sexuality.

    Results: The results showed positive attitudes such as the health personnel meant that things do not have to change just because of old age, negative attitudes such as the healthpersonnel described elderly people 's sexual needs as inappropriate. The results also showed that the care environment , procedures and policies, education and knowledge had influence on health personnels attitudes.

    Discussions: The results were discussed with the help of Virginia Henderson nursing theory of human needs. Also discussed was the risk för older people to suppress their sexual needs due to healthpersonnels judgmental attitude toward elderly sexuality.

    Abstract [en]

    The causes of the worldwide increase in asthma seen during the last decades remain largely unexplained, but lifestyle and diet are suggested to play important roles. In this follow up of a large-scale population sample in Sweden, we wanted to identify modifiable risk factors for the cumulative incidence over a 13-year follow-up period. In 1990, a self-administered questionnaire was completed by 12,560 individuals from three age groups (16, 30-39 and 60-69 years of age) in two counties of Sweden. In 2003, the eligible subjects (n = 11,282) were sent a new postal questionnaire. In total 8150 (response rate 73%) answered the questionnaire. The prevalence of asthma in 2003 had increased in all ages. In the young adults, the asthma prevalence rose from 11.3% in 1990 to 25.0% in 2003. Adult asthma onset was identified in 791 of the participants. Smoking [RR (95% CI) = 1.37 (1.12-1.68)], BMI [1.49 (1.25-1.77 per inter quartile range)], and nocturnal gastro-oesophageal reflux (GOR) [2.16 (1.72-2.72)] were significant independent risk factors for the cumulative incidence of asthma. The impact of risk factors differed between the age groups where BMI and GOR had a significantly higher impact in the middle aged and the elderly (p < 0.05). High consumption of fruit and fish was protective especially in the elderly [0.52 (0.35-0.77)]. No significant difference was found in the impact of risk factors between men and women. Weight loss, smoking cessation and a diet rich in fruit and fish may be of importance in preventing onset of adult asthma.
