    Abstract [sv]

    Bakgrund: Gynekologisk cancer drabbar ungefär 2800 kvinnor i Sverige varje år och är därmed bland kvinnor den näst största diagnosgruppen efter bröstcancer. Denna cancerform drabbar ett mycket privat område vilket innebär att den ofta omges av starka känslor och upplevs som svår att prata om.   

    Syfte: Syftet med denna studie var att beskriva kvinnors fysiska och psykosociala upplevelse av att ha överlevt gynekologisk cancer. 

    Metod: En litteraturstudie har gjorts utifrån granskning av åtta vetenskapliga artiklar. Via en manifest innehållsanalys har fyra huvudteman identifierats.

    Resultat: Att leva i en förändrad kropp: kvinnorna lever med en del kvarstående kroppsliga förändringar till följd av sjukdom och behandling. Det handlar om såväl konkreta fysiska symtom, men även om hur de här förändringarna påverkar kvinnorna, känslomässigt och psykosocialt. Ensamhet i mötet med sig själv och andra: kvinnornas upplevelse av överlevandet som en ensam process och avsaknad av stöd från närstående och sjukvård. Att skapa en ny plan för framtiden: att leva vidare efter sjukdom och behandling innebär att leva i spänningen mellan att försöka hantera det man gått igenom och söka något nytt samtidigt som man lever med en ständig oro inför framtiden. Positiv återgång till livet: sjukdomserfarenheten har inte enbart haft negativa följder utan många talar om erfarenheten som berikande och berättar om en större närvaro och uppskattning av livet. 

    Diskussion: Resultatet diskuteras i relation till Meleis transitionsteori samt relateras till tidigare forskning på området. Resultatet har visat att kvinnor som överlevt gynekologisk cancer är tvungna att hantera förändringar på olika nivåer och anpassa sig till livet på ett nytt sätt. Överlevandet kan beskrivas som en rörelse från ett tillstånd till ett annat, under vilken kvinnorna erfar såväl vinster som förluster. Att anpassa sig till livet efter cancer innebär att de måste lära sig att förhålla sig till och hantera de förändringar och omvandlingar de går igenom.

    Abstract [en]

    Background: Medical comorbidity is common in people with mental illness. There is still a strong stigma attached to mental illness. One of these could be diagnostic shadowing, a process where health professionals wrongly presume that physical symptoms are consequences of the patient´s mental illness. Diagnostic overshadowing can lead to underdiagnosis and mistreatment of the physical condition. Stigma and self-discrimination could also prevent people with mental illness from seeking treatment.

    Aim: The aim was to investigate how people with mental illness experience somatic health care.

    Method: A literature review was conducted which included twelve articles: ten with qualitative design and two with quantitative design.

    Results: Four main themes emerged: "Experience of not being taken seriously", "Longing for someone who listens", "Self-discrimination and shame" and "Experiences of barriers".

    Discussion: The results was discussed in relation to Eriksson's nursing theory, Nordenfelt theory of dignity and the consensus concepts of person, health and environment. The most distinct and common theme was the experience of not being taken seriously. The health professionals did not listen to the patients and assumed that the physical symptoms were consequences of the mental illness. This is stigma in action and it contributes to suffering and affects the patient’s sense of dignity.

    Abstract [en]

    Background: Palliative care is a type of care, which does not cure but eases the suffering caused by the disease. The goal of palliative care is not to extend or shorten the patient's life, but to relieve pain and distressing symptoms and to promote the patients’ quality of life (QOL). QOL can be understood as a subjective sense of happiness of existence, which means that the patient is experiencing life when her total life situation is consistent with her ​​wishes. A central task of the nurse in palliative care is to create conditions for improved QOL for the patient.

    Aim: The aim of this literature review was to describe patients’ perceptions’ of quality of life in the final stages of life in palliative care.

    Methods: A literature review based on ten scientific articles was used. The qualitative analysis method was used to compile the results into various theme based on the literature reviews purpose.

    Results: Five different themes were identified; to preserve social relationships, support from health care, have physical and mental well being, maintain normality and have faith in God. These themes described patients' experiences of QOL in palliative care. Being surrounded by family and friends in the last days of life was revealed as significant conditions in an increased QOL. The nurse's sensitivity and good caring with the patient was described as important experience safety and care as meaningful in the end of life. Discussions: In discussion two themes were discussed: the preservation of social relationships and support from the health care, which were the most prominent of the patients as experiences of QOL. These two themes were tied to the 6 S:n model and discussed.

    Abstract [en]

    Background: Psychosis is a large and widespread health problems in Sweden, each year 1500- 2000 people fall ill, and often causes great suffering for the individual. Religion has for people with psychosis shown to provide experience of hope, meaning, comfort and explanation. However, the patient's psychotic symptoms in the form of religious delusions, tend to make it difficult for health professionals to distinguish pathological beliefs with religious content from healthy religious thoughts. The probability to meet people from other religions and cultures is high in relation to the existing multiculturalism. We need more research to increase the possibility of caring from a holistic perspective where patients' religious thoughts and needs are included in the care process.

    Aim: To describe nurse’s experiences of meeting individuals with psychosis and their religious thoughts.

    Methods: Qualitative interviews were conducted with six nurses working in the psychiatric field. The data was interpreted through a qualitative content analysis according to Elo and Kyngäs.

    Result: The study resulted in three themes; The nurse in the meeting, Caring- to meet the needs and the nurse's experience of the patient in the meeting. The result shows how fear, insecurity and a sense of lack of knowledge of how to meet the religious needs and thoughts, and difficulties in how to distinguish healthy religious thought from religious delusions.

    Discussion: The results are discussed with Katie Eriksson holistic perspective as a theoretical basis as well as relevant scientific research. Factors such as fear, insecurity and a sense of lack of knowledge must be addressed to enable caring from a holistic perspective.

    Abstract [en]

    Background: Acupuncture has been used for thousands of years as treatment for various illness/diseases. In Sweden the method has been used primarily for treating pain. The authors want to examine acupuncture as a caring treatment by focusing on the patients experience. Longterm pain is common, wide spread and complexed problem which is hard to treat. The patient often experience unsatisfactory results with traditional treatments and medicine offered. As a nurse meets patients in pain on a daily bases in her work and her foremost task is to ease pain and it’s therefore necessary to be open to various treatment alternatives.  

     

    Aim: To examine and describe the experience of acupuncture as a nursing treatment in patients suffering from long term pain.

    Method: This analysis is a literature study consisting of 12 articles. Five of these are quantitative and seven qualitative articles have been studied with content analysis and three main themes emerged. Patients over 14 years old and articles from all the world are included. Katie Erikssons caring theory has been used as a theoretical frame of reference.

     

    Results: The majority of patients has expressed positive physical, psychological and spiritual experiences after acupuncture treatment, such as perceived relief of pain, relaxation, well-being and personal growth. Patients reported that the caring relationship and the holistic approach was two important aspects in caring. A minority of patients did not experience improvement and side-effects such as tiredness and exhaustion.

    Discussion: The placebo effect and the significance of the caring relationship have been discussed as it is unclear what the experienced relief of pain depends on. The consequences for the patients when the caring relationship is lacking is suffering from the care given. Furthermore the holistic approach showed health improving factors for the human as a whole-being. The authors believe they detect parallels between the ground principles of acupuncture and Katie Erikssons caring theory.

    Bakgrund: Varje år orsakar vårdrelaterade infektioner patienterna och deras anhöriga ett lidande, vilket dessutom kostar samhället miljontals kronor på grund av förlängda vårdtider. Detta kan i stor utsträckning förhindras om handhygienen sköts på rätt sätt. Dock har det kommit till känna att riktlinjer kring handhygien inte följs, trots att det idag finns tydliga föreskrifter och lokala riktlinjer på varje sjukhus. Man är skyldig att följa dessa riktlinjer som sjuksköterska.

    Syfte: Syftet är att beskriva orsaker till varför sjuksköterskor inte följer riktlinjer gällande handhygien.

    Metod: En litteraturöversikt enligt Friberg (2006) gjordes med nio vetenskapliga artiklar som analyserades och kategoriserades efter funnet innehåll.

    Resultat: Följande kategorier utformades i de funna resultaten; tidsbrist i relation till arbetsbörda, okunskap samt personalens inverkan på handhygien.

    Diskussion: De olika aspekterna som framkom i resultatet diskuterades och reflekterades över med hjälp av kopplingar till Nightingales teorier om omvårdnad och dess betydelse för patientens säkerhet. Anmärkningsvärt nog följer inte sjuksköterskorna handhygienrutinerna, det har framkommit tydligt och även mellan raderna att man främst skyddar sig själv och inte tänker på patienten och smittspridningen i första hand. Detta framkommer genom den okunskap de visar gentemot gällande riktlinjer trots många års erfarenhet inom vården.

    Abstract [en]

    Background: When a person has a pattern of psychoactive substance and/or alcohol use that is causing damage to health family members are affected. The quality of life of parents, siblings, children, partners and other relatives may be severely impaired. When young adults transition from child/youth to adult care it involves changes for the patients as well as for relatives. To be able to support relatives, deeper knowledge is needed about the way nurses perceive the situation of relatives.

    Aim: The aim is to investigate how nurses perceive the situation for relatives when a person is being treated in addiction care, as well as the consequences for relatives when patients are being transferred from children/youth care to adult care. 

    Method: This is a qualitative study with an inductive approach. Eight semi-structured interviews have been carried out with nurses working within addiction care. The collected data was analysed according to qualitative content analysis.

    Result: Data analysis resulted in the formulation of two categories, with two sub-categories in each. The first category describes relatives’ situation at transition from child/youth care to adult care with two sub-categories are lack of transparency and changed life situation.

    The second category describes relatives’ situation with constant worry when having a relative with pattern of psychoactive substance and/or alcohol use. This category formulated with sub-categories psychological burden and social burden.

    Discussions: The result is discussed in relation to Afaf Ibrahim Meleis’ transition theory and other relevant research. Transition theory provides support for changed life situation of relatives within addiction care, which may be important to know in order to develop nursing care.

    Abstract [en]

    Background: Shift work is common among nurses all over the world. It has been related to negative consequences like circadian rhythm disorders and health issues. Hence it is important to illustrate how shift work is affecting nurses and the care they are giving.

    Aim: To describe the consequences shift work has on nurses and their work.

    Method: A literature review based on 14 quantitative studies, collected from PubMed. The search terms were: shift work, nurse, patient safety and fatigue. The studies have been investigated and analyzed with influence by Friberg (2012a), where similarities and themes were identified.

    Results: Extended and quickly rotating shifts contributes to burnout, sleep disturbances, fatigue and elevated frequency of errors. Despite this, many nurses are satisfied with working extended shifts since it shortens their workweek. A central aspect is the opportunity to recover between shifts.

    Discussions: The authors discuss the results based on Kim’s (1987) theory on nursing domains, and the consequences of shift work on nurses, client-nurse interaction and environmental factors. Forming healthful work environments where the safety of both nurses and patients are prioritized are crucial to prevent negative consequences of shift work.

    Abstract [en]

    Background: A health damage that has been specifically related to nursing is pressure ulcers. Pressure ulcers are a complication caused by disease, care and treatment and can result in great suffering for the patient. Pressure ulcers can affect people both physically and psychosocially and is experienced, by most patients, as a serious and unnecessary complication of care. Today there are hundreds of identified risk factors for pressure ulcers described that can be divided into patient and care-related risk factors. A successful pressure ulcer prevention and treatment is to reduce or eliminate the underlying causes for the development of pressure ulcers, prevent deterioration and to treat existing ulcers. To prevent the occurrence of pressure ulcers and heal them, nurses’ knowledge and skills become important.

     

    Aim: The aim was to describe the nurses´ knowledge and attitude towards pressure ulcer prevention and treatment and how it affects the practice.

     

    Methods: The study was conducted as a literature study and was based on Willman, Stoltz och Bahtsevani (2011) methodology for reviewing scientific articles. 12 articles were chosen, three of which were qualitative, eight quantitative and one quantitative and qualitative. The analyze was conducted by Fribergs (2012) analysis regarding qualitative and quantitative research.

     

    Results: The result are presented under the four headings: Differences in nurses’ knowledge, Either you love pressure ulcers or you hate them, Lack of access to training, despite its positive impact, The attitude has a greater impact on the practice than knowledge.

     

    Discussions: The results were discussed based on the concept of health care environment, where the focus was directed towards a psychosocial component. Areas addressed include lack of access to education on pressure ulcers and what it can lead to, how the nurses’ negative attitude affect the practice and finally discussed the need to further examine of how nurses' attitudes can be improved and how they can be inspired to a greater interest in their work with pressure ulcers.

    Abstract [en]

    Background: Sexuality still includes a taboo in almost all societies, which may have contributed that the topic is relatively unexplored, despite the knowledge that sexuality is important for health and happiness. It is found that illnesses and medical conditions can have a negative impact on sexuality. In literature it is described how patients experience a lack of conversation and information about sexuality in the context of disease and medical conditions. Despite the acknowledged importance of sexuality for quality of life nurses often avoid or leave out a sexual anamnesis.

    Purpose: The purpose was to examine which factors that may facilitate or inhibit communication with patients about sexuality from the nurse's perspective.

    Method: A literature review which is based on ten scientific studies, both qualitative and quantitative, searched through the databases CINAHL, MEDLINE and PubMed. Data analysis has been done in agreement with Friberg and inspired by Granheim and Lundman.

    Results: It was found that a good nurse-patient relation and good communication skills encourage conversation about sexuality. However, it emerged mainly inhibitory factors that affect communication with patients about sexuality. The main barrier proved to be that the nurses did not take the time to discuss sexuality. The second major barrier was that nurses have a preunderstanding that patients do not expect nurses to approach sexuality. Other barriers were related to culture, lack of knowledge, the complexity of communicating about sexuality and the nurse's perception of responsibility.

    Discussion: A major barrier that might form the basis for nurses' attitudes is lack of education. A key aspect for the provision of professional nursing is a good nurse-patient relation, which requires knowledge and experience. Through training and education in how to approach sexuality the nurse can obtain the knowledge and tools that can promote communication. A prominent barrier was the comfort level, related to sexuality seen as taboo and too private to discuss. In order to address sexuality nurses themselves need to believe that sexuality is an important part of nursing care, and then are able to communicate that sexuality is an appropriate and permissible subject to discuss.

    Abstract [en]

    Background: Depression is seen as part of bipolar disorder when the individ suffers from both depressive and manic or hypomanic episodes or as a unipolar syndrome only with depressive episodes. Physical activity is defined as a body movement resulted from the muscles´ activity. It leads to increased energy consumtion. It has been linked to reducing the risk of depression, of isolation and to improving the feeling of well-being. It is not an easy task to motivate elderly to practise physical activity. The nurse has an important role in implementing physical activity in this patientgroup´s care. The psychiatric nurse´s responsiblity is to ensure that patients are offered the support that can help to their recovery.

    Aim: The purpose of this study is to describe the nurses´ perceptions of physical activity as part of the care of the elderly depressed patients who are treated within psychiatric care.

    Method: Six semi - structured interviews were done with nurses working on a ward specialized on taking care of elderly suffering from mental illnesses. Inductive content analysis inspired from Granheim och Lundman (2012) has been used to analyse the interviews.

    Results: Two categories resulted from the analysis: "support based on promoting physical activity" and "health impact". Three subcategories (practical, emotional and social support) are included in the category "support based on promoting physical activity". Subcategories "physical impact", "psychological impact" and "social impact" belong to the category "health impact".

    Discussions: The results were discussed in relation to Barker´s tidal model (2005) with emphasis on three of his concepts (self-domain, others´ domain and the world domain). The psychiatric nurse has the opportunity to promote physical activity for these patients by offering them practical, emotional and social support.

    Abstract [en]

    The aims of this thesis were to explore nurses' experiences of stress in the ICU (I), to analyze experiences of moral concerns in intensive care nursing from the perspective of relational ethics (II), to describe the synthesis of the concept of moral stress and to identify preconditions for moral stress (III) and to analyse and describe lived experiences of support in situations characterized by critical care situations and moral stress in intensive care (IV).The design was exploratory and descriptive. Material in studies I, II and IV consisted of interviews with intensive care nurses (10 head nurses and 26 staff nurses) employed in general, thoracic and neonatal intensive care units in five hospitals located in different parts of Sweden. The material in study III data from two studies of professional issues in nursing were used for the analysis: one concerned psychiatric nursing and the other was the previously referred study I.In study I qualitative content analysis and descriptive interpretation was used in the analysis. The main theme 'stress induced by dissonant imperatives' formulated in the analysis. Dissonant imperatives are composed of the four sub-themes: 1) controlled by the working situation - needing to be in control, 2) constrained by prioritisation - wanting to do more, 3) lacking authority to act - knowing that something should be done, and 4) professional distance - interpersonal involvement. In study II qualitative content analysis and descriptive interpretation were used in the analysis. A main theme was formulated, 'caring about-caring for: tensions between moral obligations and work responsibilities in intensive care nursing'. Five sub-themes were formulated 1) believing in a good death, 2) knowing the course of events, 3) feelings of distress, 4) reasoning about the physicians and 5) expressing moral awareness. In the study III a hypothetical-deductive method was used. The findings indicate that moral stress is independent of context-given specific pre-conditions: 1) nurses are morally sensitive to the patient's vulnerability, 2) nurses experience external factors preventing them from doing the best for the patient, and 3) nurses feel that they have no control over the situation. In the study IV an interpretive method was used. The first level of analysis of data identified contextual factors, such as type and purpose of support and working conditions. Thereafter five tentative interpretations were revealed: 1) receiving organised support is a matter of self-determination, 2) whether to participate or to be off duty is experienced mutually as exclusive, 3) dealing with moral stress is experienced as a private matter, 4) colleagues managing moral stress serve as models in stress support, and 5) not being able to deal with moral stress urges one to seek outside support. A comparison of these interpretations identified three major themes: availability, accessibility and receptivity of support. The main interpretation of data was: "lived experience of moral stress support involves an interconnectedness between structural and existential factors".A comprehensive understanding was formulated using the four studies (I, II, III and IV). Moral stress was found to be influential on the caring competence. Conflicts between different competences were found leading to a shift in focus away from the patients leading to a possible decrease in the caring competence. Moreover, the subtle resistance among nurses toward participation in organized moral stress support may obstruct the development of nurses' caring competence. Accordingly, imbalance, due to moral stress, between different competences hinders the development of collectively shared caring competence.

    The aim of this study was to analyse and describe lived experiences of support in situations characterized by critical care situations and moral stress in intensive care nursing. An exploratory interpretative study was conducted. Data consisted of interviews with 36 nurses from different types of intensive care units. The first level of analysis of data identified contextual factors, such as type and purpose of support and working conditions. On the next level of analysis five tentative interpretations were identified: (1) receiving organized support is a matter of self-determination, (2) whether to participate in organized support or to be off duty are experienced as mutually exclusive, (3) dealing with moral stress is experienced as a private matter, (4) colleagues managing moral stress serve as models in stress support and (5) not being able to deal with moral stress urges one to seek outside support. A comparison of these interpretations identified three major themes: availability, accessibility and receptivity of support. The main interpretation and conclusion were: lived experience of moral stress support involves an interconnectedness between structural and existential factors. Thus, adequate moral stress support presupposes an allowable professional climate and access to caring supervision.

    Abstract [en]

    BACKGROUND: Intensive care units are characterized by heavy workloads, increasing work complexity and ethical concerns related to life-and-death decisions. In the present study, it is assumed that there is a relationship between moral stress, support and competence for nurses in intensive care units. AIM: To analyse and describe the theoretical relationship between moral stress and support on the one hand and competence on the other, in the context of intensive care. METHOD: A form of qualitative secondary analysis based on the findings from three original studies. In the analytic process a theory on professional competence was used. FINDINGS: The findings suggest that imbalance due to moral stress between different competences hinders the development of collectively shared caring competence. CONCLUSIONS: Moral stress cannot be totally eliminated in the intensive care unit. But moral stress is not only a problem. It can also become a driving force to stimulate competence.

    Abstract [en]

    Background: A few studies has been conducted where interventions with a trained care dog is used in the care of the old. The studies show both positive and less positive effects on physical, mental and emotional behavior in people with dementia. A certain effect has also been seen on verbal and nonverbal communication in people with dementia.

    Aim: The aim of the study is to describe how verbal communication is embodied in a person with a severe dementia in meeting with a trained care dog.

    Methods: One case study was carried out where the material has been analysed with a descriptive qualitative content analysis. Meaning units have been found and coded in order to create subcategories, categories and themes that brings a new understanding and wholeness of the phenomenon being studied.

    Results: Two themes with five categories and 15 subcategories could be discerned. The themes with categories are; To be present in the moment with categories "Attentive" and "Understand the situation" as well as The importance of interaction with others, with categories "Security", "Awareness" and "Social skills".

    Discussions: To be in the moment and to participate in social context were important aspects of the study when the woman meet the care dog. Insights to emergence of memory and that verbal communication is preserved in a person with severe dementia. The woman can give us another piece of the puzzle in understanding what is happening with verbal communication when having severedementia. Seeking to understand the situation is something that emerged and the woman has many questions and observations in order to create an understanding of the situation and the environment she finds herself in.

    Abstract [sv]

    Bakgrund: Att vara närstående till en person med demenssjukdom kan vara påfrestande. En förändrad situation uppstår med många praktiska frågor där den kvinnliga närstående ofta förväntas gripa in. Ofta ligger ansvaret på kvinnan, att se till att vardagen fungerar för den närstående med demenssjukdom. Bördan som kvinnorna upplever kan bland annat leda till depression och stress.

    Syfte: Syftet är att beskriva kvinnors upplevelse av att vara närstående till en äldre person med demenssjukdom.

    Metod: En tolkning av de självbiografiska böckerna som inspirerats av den narrativa metoden gjordes för att få en ökad förståelse för kvinnans upplevelse av att vara närstående till en person med demenssjukdom. Livsvärldsperspektivet användes som teoretisk referensram. Genom att analysera berättelserna som kvinnorna skrivit utifrån sina upplevelser kan kunskaper lyftas fram och därmed ge sjuksköterskan verktyg för att kunna bemöta kvinnorna utifrån deras specifika behov.

    Resultat: Kvinnorna upplevde en sorg och saknad av den förändrade relationen de haft till sin make, mamma eller syster. Kvinnorna upplevde en börda i att vara den som ansvarade för att deras närstående fick det stöd, den hjälp och vård som de behövde. De upplevde en ensamhet och ett lidande samt avsaknad av välbefinnande i det dagliga livet.

    Diskussion: Kvinnor som vårdade en närstående person med demenssjukdom var i behov av mer stöd från den professionella vården och få hjälp med samordning av de olika instanserna som utförde vård och omsorg. Dessa insatser som sjuksköterskan kan göra för kvinnorna kan leda till ökad kvalitet på den vård som de utför samt även öka välbefinnandet hos kvinnan som vårdar i hemmet och hos personen som blir vårdad.

    Abstract [en]

    Background: Abortion implies that the pregnancy is interrupted. According to the World Health Organization approximately 46 million induced abortions are carried out each year. Different methods of abortion can be used such as medical and surgical methods. The medical method especially affects a nurse’s experience of the procedure. Most likely, a woman undergoes the abortion process only once in her lifetime, while the nurse may face the experience several times a week. Induced abortion can be morally difficult for nurses and can generate ethical dilemmas between the woman´s right and the fetus value of protection.

    Aim: The aim of this literature review was to describe nurses experiences when they participate in the implementation of induced abortion.

    Method: A literature review has been conducted, where ten caring sciences articles were studied. The articles were all of qualitative design.

    Results: The analyses of the results were divided into two themes: nurses approaches to abortion. Feelings that could occur in relation to abortions were guilt, shock, fear, anger and sadness.  Most nurses used self-preservation to shut out emotions. The most demanding and challenging situations. All nurses were affected emotionally, especially in relation to repeated and late abortions. It could also occur an ethical conflict between a woman´s right and the fetus value of protection. It was therefore important for the nurses to have the opportunity to discuss and reflect on their experiences.

    Discussions: The discussion addressed: feelings associated with late and repeated abortions as the nurses need support to care for women undergoing an abortion. The results were discussed in relation to Jean Watson theory of nursing.

    Abstract [en]

    Background: Sleep is an important part of a person’s life, you sleep away a third of your lifetime. With the natural aging there will be changes in the sleep pattern. You will become more sensitive to surrounding stimuli, wake up easier at night and sleep less hours. On a ward, there are a lot that can affect sleep, especially for elderly. Sound that you don’t recognize, an unknown bed to sleep in as well as other people in the same room. All these factors can make you get sleep disorders during sleep in a ward.

    Aim: The purpose of this literature review was to describe factors impact on older adults and older peoples sleep in a ward.

    Methods: In this literature review the databases CINAHL with full text and Medline have been used. A total of eleven articles were reviewed, eight articles had quantitative methods and two had qualitative methods, the last article were a mixed method with both qualitative and quantitative design. From the results, the authors had complied themes.

    Results: Five themes noticed: sleep in a ward, where the sleep is affected by the unfamiliar environment. Inside factors that affect sleep, where the emotional state of the patient caused by worry, disease and absence of relatives can affect the sleep. Environmental factors that disturb sleep, where light, sound and staff interactions contribute to disrupt sleep. Differences between countries and gender differences that affect the sleep, where men and women's sleep quality are different between countries. Noise reducing measures for sleep, where scheduled silence (Quiet Time), light- and sound reduction programs are important factors to promote quality sleep.

    Discussions: The methodological discussion we have discussed about the choices of articles and countries may have negatively affected the result, but also discusses why we chose to use them. The results discussion, we had discussed Florence Nightingale's theory of environment and sleep related to the result, and what has not been included in the results.

    Abstract [en]

    Background: Misuse of alcohol and narcotics is a socially and global problem. In the year of 2012, 29 million people suffered from an addiction of alcohol and narcotics. In contact with care, people with addiction has described negative experiences and attitudes of health care professionals. Stigma against people with addiction is occurring which may influence the care they receive.

    Aim: To describe nurses' attitudes and experiences in meeting with people with alcohol and drug abuse in somatic care.

    Method: To answer the aim of the study the authors conducted a literature review where nine articles of qualitative approach and one article of quantitative approach were analysed.

    Results: This literature review is presented in five main themes where three main themes have associated subtheme: The first theme is Nurses fear and substance misuser’s lifestyle with associated subtheme nurses' ambivalence. The second theme is responsible or not responsible for their addiction. The third theme consists of not having the knowledge and fourth theme consists of nurse’s mistrust with associated subtheme consequences of mistrust. The last theme is acting out and violence creates stress and anxiety associated with subtheme consequences of acting out and violence.

    Discussion: The result has been discussed from Erikssons caritative theory with focus on the concept of suffer of care. The nurses who had negative experiences of people with addiction problems often had negative attitudes towards these people. The negative attitudes that some nurses had yielded in turn implications for the care they gave. Nurses felt mistrust in the meeting with people who had an addiction which affected the therapeutic relationship. Lack of knowledge was an impediment to care for these patients and the nurses vulnerable position led to a detached approach.

    Abstract [en]

    Background: Anorexia nervosa means severe weight loss, which is a consequence of fear of gaining weight. A large part of the nursing consists of controlled treatment of physically symptoms. That can lead to a struggle of power due to the fact that people with anorexia nervosa neither wants to be helped or get well. This challenges the creation of care relationship. A working health care relationship is crucial for the recovery.

    Objective: To describe patients and nurses experiences of the health care relationship in nursing of anorexia nervosa.

    Method: The study is based on a qualitative literature review. The information was retrieved from the Cumulative Index to Nursing and Allied Health Literature, CINAHL, database. The material was analyzed using a content analysis.

    Results: From a patient and nurse perspective two main and eight sub themes emerged regarding experiences of nursing in treatment and relationship promotional factors in nursing. The first main theme that emerged was removal of control and responsibility, which has the following sub themes; experiences of:  treatment, the health care relationship, the significance of communication, the significance of availability and individualized care. The second main theme was to restore control and responsibility, with the following sub themes; experiences of: the significance of self-determination, the significance of knowledge and the significance of support.

    Discussion: The nurse should take in consideration individual differences in nursing patients with anorexia nervosa.The controlled nursing measures, with focus on the physically symptoms, contributes to that the individual becomes neglected. With the help of Travelbees nursing theory the authors discuss the importance of creating a health care relationship to promote the patients autonomy and integrity.

    Abstract [en]

    Health care often involves ethically difficult situations that may disquiet the conscience. The purpose of this study was to develop a questionnaire for identifying various perceptions of conscience within a framework based on the literature and on explorative interviews about perceptions of conscience (Perceptions of Conscience Questionnaire). The questionnaire was tested on a sample of 444 registered nurses, enrolled nurses, nurses' assistants and physicians. The data were analysed using principal component analysis to explore possible dimensions of perceptions of conscience. The results showed six dimensions, found also in theory and empirical health care studies. Conscience was perceived as authority, a warning signal, demanding sensitivity, an asset, a burden and depending on culture. We conclude that the Perceptions of Conscience Questionnaire is valid for assessing some perceptions of conscience relevant to health care providers.

    Abstract [en]

    Stress among healthcare students is a growing problem. As self-comfort is assumed to be a way of coping with stressful emotions, the aim of this study was to describe the patterns of self-comforting actions that healthcare students usually use in distress. One hundred and sixty-eight healthcare students volunteered to write down accounts of what they do when they comfort themselves. Their accounts were analysed using qualitative content analysis. The findings reveal two themes: Ingressing and Transcending. Ingressing comprises the sub-themes Unloading, Distracting, Nurturing oneself, Withdrawing and Reassuring. Transcending comprises the sub-themes Opening up and Finding new perspectives. These findings are in line with some stress-reducing strategies described in the literature on stress management. Winnicott's theory about the phenomenon of transition is used to interpret the findings. In the light of Winnicott's theory, self-comforting measures can be comprehended as the ability to transfer early childhood experiences of being nurtured and comforted into well-adapted strategies to effect relaxation and gain strength.

    The aim of this study is to illuminate the meaning of encounters with a troubled conscience among psychiatric therapists. Psychiatric care involves ethical dilemmas which may affect conscience. Conscience relates to keeping or losing a sense of personal integrity when making judgments about one's actions. Ten psychiatric therapists were interviewed in June 2006. The interviews were tape-recorded, transcribed verbatim and interpreted using a phenomenological-hermeneutic method. Two themes 'Facing inadequacy' and 'Struggling to view oneself as being 'good enough'' are presented. In the therapists interviewed, awareness of their use of power, a sense of powerlessness and a sense of blame gave rise to feelings of betrayals and shameful inadequacy. By sharing their inadequacy with co-workers, they managed to endure the sense of their inadequacy which otherwise would have threatened to paralyse them. Finding consolation in sharing wearing feelings, becoming realistic and attesting their worthiness, they reached reconciliation and found confirmation of being good enough. The findings are interpreted in light of Lögstrup's ethics of trust, according to which conscience alerts us to silent but radical ethical demand and the risk of self-deception.

    Abstract [en]

    Background: Anorexiais a disease that often emerges in adolescence. Characteristic symptoms are significant, deliberate weight loss along with strong control needs regarding food and movement that occupies most of the person’s life. Without treatment, the condition can be life-threatening. Alliance and a good care relationship is considered to be of great importance for a successful treatment outcome, but at the same time difficult to establish with the patient group.

    Aim: To describe what promotes a caring relationship between nurse and patient with anorexia, which strengthens the patient's health process.

    Method: Narrative meta-synthesis, which includes 14 articles with a qualitative perspective. Analysis has been inspired by Evans (2002) descriptive synthesis process.

    Results: The results are presented in four main themes: Nurse's approach; the relationship bases; Focusing on individual; Knowledge, and in 10 sub themes. These themes were considered to promote a caring relationship that affected the patient's health process positively.

    Discussions: The results are discussed with the support of the theory Tide model's assumptions and concepts, as well as in previous research based on a person-centered and knowledge- focused perspective.

    Bakgrund: Forskning visar att det i Sverige blir allt fler äldre människor; samtidigt är dagens sjukvård inte organiserad för att möta deras behov. Äldre har ofta ett stort behov av omvårdnad och det är viktigt att sjuksköterskan arbetar personfokuserat relaterat till detta. Det sker idag en åldersdiskriminering av äldre och sjuksköterskans personliga egenskaper, såsom hennes attityd och bemötande, kan påverka vårdens kvalitet och hur den äldre upplever sin hälsa. Där av tydliggörs vikten av en god vårdrelation, med respekt för individens autonomi, integritet och värdighet.Syfte: Syftet är att beskriva vilka faktorer som kan ha betydelse för sjuksköterskans bemötande av äldre.Metod: Litteraturstudie baserad på tio vetenskapliga artiklar. Litteratursökningen genomfördes under mars och april månad, år 2011, i de elektroniska databaserna CINAHL, Medline och Pubmed. De vetenskapliga artiklarna härstammar från; Sverige, Norge, Finland, Australien, Kanada och Storbritannien.Resultat: Vid analys av de vetenskapliga artiklarna hittades två genomgående huvudkategorier, med respektive subkategorier. Huvudkategorierna var; yttre faktorer med subkategorierna; organisering av vården, tidsbrist och vårdmiljön, och inre faktorer med subkategorierna; kunskapsnivå, attityder och Sjuksköterskans erfarenheter av att vårda äldre.Diskussion: I resultatet av den här studien framkom att många av de identifierade kategorierna påverkade varandra, exempelvis är organisatoriska faktorer avgörande för vårdmiljön och sjuksköterskans tillgängliga tid. Vilka också påverkar sjuksköterskans inre faktorer. Organisationen av vården påverkade sjuksköterskans bemötande gentemot äldre både positivt och negativt.     Background: The World Health Organization (WHO) defines palliative care as a total care of the patient and their relatives, provided by a multi professional healthcare team. Palliative care is an area where the next of kin's role is particularly important because when a family member suffers from an illness it affects the whole family. Many people with serious illness wish to be cared for at home at the end of life. When a person is cared for at home, often the next of kins take the caring role and the responsibility over their relative's care. The next of kins involvement in the care of their loved one can affect them physically and mentally.

    Aim: To describe the next-of-kin's experiences of caring for a loved one at the end of life at home.

    Method: The literature review was based on eleven scholarly articles with a qualitative approach. Articles were gathered through database search in CINAHL complete with full text and Medline.

    Results: The result showed that the next of kins experienced a responsibility to care for their loved one. Taking responsibility influenced the caregivers own health and social activities. Work and hobbies were limited since focus was on their loved one's needs. To witness the deterioration of their loved one at the end of life was perceived particularly difficult. Support from the family, friends and health care professionals facilitated them to manage the care giving. However, the next of kin experienced insufficient support by healthcare when it came to information, guidance and preparedness for the caregiver role.

    Discussions: The result was discussed in relation to Meleis transition theory, because the next of kins were seen to go through a transition when they were caring for an ill loved one. Furthermore, the nurse's role was discussed with a focus on how to help the next of kin to undergo a healthy transition.

    Background: In the background section the following terms are described: Palliative care and holistic view, care at the end-of-life, ethical problems in the end-of-life care and ethical problem and ethical dilemma.

    Aim: The purpose of this literature review was to describe nurses’ experiences of ethical problems in the end-of-life care of patients.

    Method: A literature review was selected as a method in this study based on eightscientific articles. Articles were reviewed and analyzed critically by the author. Travelbees’ theory (1971) “human- to- human relationships” was selected as a theoretical basis.

    Results: The result presents six themes as follows: Decision-making, ineffective treatments and therapies, insufficient communication, the lack of cooperation, inadequate respect for patient’s autonomy and uncertainty in caring role. These themes present how nurses deal with end-of-life care and in which situations ethical problems arise.

    Discussions: The result was discussed in relation to Travelbees’ theory (1971) “human-to-human relationships”. Nurses’ different experiences according to their different responsibilities such as insufficient communication and cooperation, decision-making processes, uncertainty in caring role and inadequate respect for patients’ autonomy were discussed.
