    Abstract [en]

    Background: In Sweden 160 000 people live with a dementia diagnosis today. Dementia can give several behavioral and psychological symptoms. Research has shown that music can comfort and calm a person. Long term music memory has shown to be intact in people with dementia.

    Aim: To study the impact of music on people suffering from dementia.  

    Method: A literature review according to Friberg's method where the result is based on eleven scientific articles; five qualitative and six quantitative original articles. Databases used were Cinahl and PubMed with the search terms dementia, music, nursing and singing.

    Results: Music at mealtime can decrease agitated behavior. Music during morning care sessions can increase the communication between caregiver and people suffering from dementia and also increase the understanding of the everyday activities. Individually listening to music can decrease agitation and increase communication with the person suffering from dementia.

    Discussion: The results are discussed in relation to the consensus concept caring, where the focus for the discussion was how music can be used as a nursing intervention.

    Abstract [en]

    AIM: This study aimed at evaluating the outcome of implemented evidence-based clinical guidelines by means of surveying the frequency of thrombophlebitis, nurses' care, handling and documentation of peripheral intravenous cannulae. BACKGROUND: Peripheral intravenous cannulae are frequently used for vascular access and, thereby, the patients will be exposed to local and systemic infectious complications. Evidence-based knowledge of how to prevent these complications and how to care for patients with peripheral intravenous cannula is therefore of great importance. Deficient care, handling and documentation of peripheral intravenous cannulae have previously been reported. DESIGN: A cross-sectional survey was conducted by a group of nurses at three wards at a university hospital before and after the implementation of the evidence-based guidelines. METHOD: A structured observation protocol was used to review the frequency of thrombophlebitis, the nurses' care, handling and the documentation of peripheral intravenous cannulae in the patient's record. RESULTS: A total of 107 and 99 cannulae respectively were observed before and after the implementation of the guidelines. The frequency of peripheral intravenous cannulae without signs of thrombophlebitis increased by 21% (P < 0.01) and the use of cannula size 0.8 mm increased by 22% (P < 0.001). Nurses' documentation of peripheral intravenous cannula improved significantly (P < 0.001). CONCLUSION: We conclude that implementation of the guidelines resulted in significant improvements by means of decreased frequency of signs of thrombophlebitis, increased application of smaller cannula size (0.8 mm), as well as of the nurses' documentation in the patient's record. RELEVANCE TO CLINICAL PRACTICE: Further efforts to ameliorate care and handling of peripheral intravenous cannulae are needed. This can be done by means of increasing nurses' knowledge and recurrent quality reviews. Well-informed patients can also be more involved in the care than is common today.

    Barns och ungdomars psykiska ohälsa är ett ofullständigt utforskat område i Sverige. Mycket tyder dock på att den psykiska ohälsan ökar bland unga vuxna, vilket gör att det finns all anledning att undersöka hur just denna grupp bäst kan bli hjälpt. Syftet med denna studie har varit att få en större förståelse för hur återhämtningsprocessen från psykisk ohälsa kan se ut för denna grupp: vad som är verksamt i processen och vilka faktorer de upplever som viktiga, pådrivande och hjälpande. Detta har undersökts i fem kvalitativa intervjuer med unga vuxna mellan 23 och 27 år som lidit av psykisk ohälsa men som idag mår bra igen. Ingen av dem har vårdats på institution för den psykiska ohälsan, men alla har fått professionell hjälp. Intervjuerna har analyserats med hjälp av teori inom återhämtningsforskningen och bearbetats med hjälp av kategorisering. Resultatet lyfter fram vikten av att få en tydlig reaktion på sin psykiska ohälsa; att man blir sedd och bekräftad av någon som också vågar agera och ta ställning. Det visar även att behandlarens ”regelbrott” kan ha en positiv inverkan, att sociala kontakter är viktiga under processen samt att man önskar en större tillgänglighet till vården. Slutsatsen är också att dessa behov för unga vuxna ska ses i ljuset av närheten till deras barndom och ungdom. Detta sammantaget ställer krav på förhållningssätt och tillgänglighet hos oss som möter unga vuxna i det sociala arbetet, från socialtjänst och skola till terapimottagningar och fritidsverksamheter. Det får också implikationer på det strukturella planet, till exempel hur ansvar och resurser fördelas mellan olika aktörer. Om detta behövs mer forskning, samt om unga vuxna explicit, eftersom de nu som grupp blir allt större, men fortfarande är otydligt definierade i både statistik och debatt, men också inom vården.

    Abstract [en]

    Background: Previous studies have shown that patients who were involuntarily treated described the care as problematic when others were taking decisions for them and the staff didn’t listen or talked to them. According to The Law of psychiatric Compulsory Care of 1991, coercive measures should be used with the highest regard considering the patient. Nurses can feel guilty about exposing the patient to coercive measures and some believed that there are no alternatives to it.

    Aim: To describe the patient’s experience of compulsory treatment and coercive measures.

    Methods: A literature review with a qualitative approach. Eleven scientific articles, published between the years 2000-2012, were used in the study. The studies were carried out in Australia, Finland, Canada, The Netherlands, Norway, Great Britain and Sweden. The studies were analyzed and the result was categorized.

    Results: Staff was described as ignorant and disrespectful and the patients wanted genuine regard. Staff exercised power which, for the patients, resulted in an experienced loss of control. Patients reported failure in communication, information, participation and were not having an adequate contact with the staff. Meanwhile some patients narrated that their care was good and offered them a sense of security. The patients’ experiences resulted in four main categories and six sub categories.

    Discussions: The definition of autonomy, by Tännsjö, was chosen to be the theoretical framework and was used in the discussion. Information, decision-making and the power of the staff were some of the topics which were discussed.

    Abstract [en]

    Background: Nurses are expected to provide care while neither preconceptions, own values, nor emotions effects caring relation. The general nurse´s role in abortion care has been increasing with the increased number of medical abortions, which puts a greater responsibility on nurses. The nurse´s dilemma is to provide high quality care to the women being at the same time responsible to take care of the fetus after the abortion. Ethical, moral and religious aspects of abortion care has always existed being a challenge to navigate between the exert of profession, current abortion law and ethical and moral positions.

     Aim: To describe the ethical problems that nurses encounter in the care of women undergoing abortion, and how they deal with and reflect on their responsibility in nursing.

     Method: Literature review of ten qualitative articles according to Friberg (2012) was performed.

     Results: The literature study results in issues; what moral/ethical problems nurse experiences in abortion care, the nurses´ experiences in dealing with this problems and what support and what strategies are needed in order to process difficult situations within the abortion care. The study resulted in four main themes; Ethical aspects regarding termination of pregnancy, Factors that influence how the nurses deal with their responsibilities in nursing, Strategies for dealing with the responsibilities in nursing and The nurse's need for support.The nurses were in great need to reflect on their demanding work in abortion care in order to preserve their own health and to pursuit a good person-centered care. 

     Discussions: The discussion based on the themes that emerged from the results was performed using Peplaus (1991) theory of the interpersonal process between nurse and patient and Sherwins (1998) feminist ethics. The discussion focuses on how the interpersonal relationship, the opportunity for reflection and the importance of a feminist approach may improve the care relationship and nurses´ management of difficult experiences in abortion care.

    Abstract [en]

    Background: Sexuality and sexual health are two concepts that belong together. Sexuality means proximity and contact while sexual health means that it is safe and secure to perform sexual activities. Cervix is a part of women's inner reproductive organs. Cervical cancer is globally seen as one of the most common forms of cancer amongst women where pain is a late symptom of cervical cancer. Mortality is higher in developing countries than industrialized countries. Human papillomavirus, HPV is the most common cause of cervical cancer, where surgery, cytostatic and radiation therapy are different types of treatment. Cytological changes do not always mean cancer. Getting a cancer message can induce emotional reactions such as anxiety, anger and concern. At different coping strategies; Active and passive, the woman can adapt to the new life situation. A cancer diagnosis can develop reactions that affect both the physical and the mental health.

    Aim: The purpose was to highlight women’s sexual health after diagnosis of cervical cancer.

    Method: This literature review was compiled by collecting material from previous research within a healthcare knowledge area. Ten scientific articles related to cervical cancer and sexuality are the basis for the outcome. The material was reviewed and analyzed according to Fribergs method and finally summarized into mainthemes and subthemes in a literature review.

    Results: The result is presented in three main themes; Physical aspects, Mental aspects, and Social aspects. The results showed that cervical cancer affected women’s sexuality and sexual health. Positive experiences expressed by the women were an increased estimation on sexual activity after the disease. While negative experiences that were reported was a fear of performing sexual activities where dyspareunia was a cause. Sexual barriers affected the sexual function and created a concern among the women. The treatment of cervical cancer affected the woman through various bodily changes and discomfort during sexual intercourse.  

    Discussion: The physical changes that the woman experience is affecting her sexuality and identity. A need for knowledge about cervical cancer related to sexuality was noted in the women, where the nurse's responsibility is to provide women with this information. Virginia Henderson's behavior theory and the Cancer center care program may be a support for the nurse.

    Abstract [en]

    After the 2016 election of president Duterte, the Philippines have been actively fighting a war on drugs inside their own country according to the new policies developed by the newly chosen regime. These policies have led to the several thousands of dead in police operations which are by many called out as extrajudicial executions. The purpose of this study is to research the experience of urban people living in a society pervaded by a war on drugs with the goal of contributing to the understanding of how they handle the situation and how it affects their lives. The authors have spent two months in the Philippines, mainly in the Metro Manila area to gather the data for the study through mainly participant observation and semi-structured interviews for an ethnographic study. The results show who are being targeted in the war, how people experience it and create labeling and stigma according to the societal rules and norms as well as how people cope with difficult circumstances out of their control.

    Abstract [en]

    Background: The goal of palliative care is to facilitate patients and their family member’s quality of life through holistic views and by working in coherence with four cornerstones: symptom relief, family support, teamwork and communication. It is more common that patients with incurable diseases want to be cared at home at the end of life. The family’s role is challenging in many ways when they care for and support their relatives at home at the end of life. These carers often need emotional or practical support from health care professionals in order to achieve good health but also for their relatives to have a good and dignified death.

    Aim: To describe the relatives’ experiences of caring for a loved one at home at the final stages of life and to develop nurses' knowledge and effort to provide appropriate support.

    Method: Data were collected from the databases CINAHL Complete and Medline with full text constraints and Peer reviewed as well as a demarcation between the years of 2003 to 2015. Ten qualitative studies were selected and analyzed.

    Results: The results showed that relatives felt a great responsibility and burden when caring for their loved ones at home at final stages of life. It affected their work, income, and health and leisure time in negative manor. They also experienced a feeling of inadequacy, isolation and loneliness, due to the lack of support from other family members, friends as well as from the health care system. Even though, care giving attributed was experienced as negative the relatives also felt a validity, satisfaction and meaningfulness of caring for their loved ones.

    Discussion: The results were discussed on the basis of Roy’s adaptation model and to the consensus concept of health. Relatives need different types of support in order to adapt more easily to their role as carers and to promote their health. Furthermore, also discussed the relatives experienced lack of support from family members, friends and the health care professionals. The nurse`s supporting role can have a significant impact to how families can adapt to the caring role and to promote their health, quality of life and well-being.

    Abstract [en]

    This study intends to investigate how young women, who have been subjected to honor-related violence and oppression, experience the support of the social services based on the power perspective and the KASAM theory. We have in the study used qualitative methods to conduct semi-structured interviews. Eight young women who had been in contact with the social services were interviewed.

    Previous research has primarily studied the client's experience of contact with the social services. Studies that concern women who have been subjected to honor-related violence and oppression are very limited. Research in this area should therefore be sought.

    The study's results showed that young women's expectations did not match the experience of contact with the social services. The results analysis showed that the social services support and assistance efforts are far too short-term where women are left behind and are expected to handle their difficulties on their own.

    Abstract [en]

    Background: Depression is one of the biggest and fastest growing causes of ill-health in the world. Depression strikes a persons life from aspects like reduced health and function and causes also social consequences. Care of depression today focuses much on the patient, where treatment, nursing and therapy occupies most of the space, and simultaneously family members are left in the shadow of the patient. Also there is an uncertainty among care providers, when trying to determine whether a patient´s symptoms have a somatic cause. This can make it more difficult for the patient to get the right treatment, which can impair the somatic state of health and lead to more suffering.

    Aim: To illuminate experiences of being a family member to a person having a depression.

    Method: The study was carried out as a literature review with help from ten scientific articles that answered to the intention of the study.

    Result: The result showed that family members experience a lot of stress and worry in connection to being related to someone affected by depression. Family members as well lack information that could help them promote both their related and their own health. Family members feel uneasiness with the future and their relateds´ present state. At the same time it was found that family members had a large role in the patient´s life, treatment and nursing.

    Discussion: The result was discussed on the basis of SOC (sence of coherence), and the concept of consensus health. The authors show a connection to how health is affected on the basis of SOC. The results confirmed among other things that information to family members is a master key to be able to support both the patient and family members and increase understanding of the disease.

    Abstract [en]

    A study was made on 150 clinically demented patients presenting at autopsy at Umeå University Hospital in Sweden. In 90 of the cases dementia was considered to be primary in nature and of these forty six per cent (41 cases), fulfilled both the clinical and histopathological criteria for the diagnosis of Alzheimer's disease/Senile dementia of Alzheimer type (AD/SDAT). The families of these 41 AD/SDAT cases were then studied, and a family history obtained through interviews with multiple family informants and from civil and medical records. Additional diseased family members suffering from progressive dementia (multiplex families) were observed in 12 probands out of 41 (29%). Multiplex families exhibited similar clinical and histopathological characteristics as simplex families containing a single affected individual. The secondary cases in the multiplex families exhibited similar demographic and clinical characteristics as the probands. 39% of the multiplex and 14% of the simplex cases had an early age of onset of the disease, that was under 65 years. The overall prevalence of progressive dementia disorders in the 41 families was 5.9%. The prevalence of a progressive dementia disorder was 11% in the multiplex families (14% for the early onset cases) and 3.5% in the simplex families (2% for the early onset cases). The prevalence of progressive dementia disorder for family members who had passed the mean age of the onset of the disease for their family, was 45% for multiplex and 18% for simplex families. Furthermore the incidence rate for dementia was significantly higher (p < 0.005) in multiplex families (5.5 per 1,000 person years) when compared to simplex families (2.5 per 1,000 person years). No differences could be seen in parental age at birth of the diseased when comparing the two sets of families. However in multiplex families the duration of the disease was significantly (p < 0.025) shorter, in subjects with parental age at birth over 35 years compared to those with a parental age under 35 years. The multiplex families contained significantly (p < 0.025) larger sibships; and showed a significantly lower age of onset for the disease (p < 0.001), and a significantly longer duration of disease (p < 0.05) compared to the simplex families. A significant intra familial correlation of age at disease onset was observed in both sets of the families.

    Bakgrund: Cancer är den näst största folksjukdomen och den näst vanligaste dödsorsaken efter hjärt-kärlsjukdomar. Prevalensen av insjuknandet ökar för varje år och innebär därmed att allt fler unga vuxna drabbas. Forskning kring unga vuxnas upplevelser av cancer är undermålig samtidigt som definitionen av unga vuxna är vag. Detta gör att det idag inte finns ett tydligt och hållbart behandlingsprogram för denna patientgrupp. Unga vuxna befinner sig i en fas i livet där stort fokus ligger på att utvecklas och att upptäcka världen. Att som ung vuxen drabbas av en livshotande sjukdom som cancer innebär därför en unik upplevelse och krisreaktion. För att sjuksköterskor ska ha möjlighet att ge de drabbade personcentrerad vård av god kvalitet, krävs en bättre förståelse för deras specifika situation. Syfte: Syftet var att belysa unga vuxnas upplevelser av att leva med cancer. Metod: En litteraturöversikt gjordes i enlighet med Fribergs metod. Resultatet är baserat på tio kvalitativa, vetenskapliga artiklar som beskriver unga vuxnas upplevelser av att leva med cancer. Artiklarna analyserades genom tematisering och kategorisering. Sökningarna gjordes i databaserna CINAHL Complete och PubMed. Resultat: Fyra teman identifierades: Att drabbas av cancer som ung vuxen, Behov av stöd, Identitetsförändringar och Behov av att ta kontroll. Samtliga teman beskriver upplevelser som är typiska för unga vuxna med cancer. En stor del av resultatet innefattar känslor och frågor av existentiell karaktär. Diskussion: Metoden diskuteras utifrån litteraturöversiktens styrkor och svagheter. I resultatet diskuteras framkomna teman med hjälp av ett livsåskådningsperspektiv, detta för att skapa en djupare förståelse för den unika situation som unga vuxna ställs inför i samband med cancer.

    Bakgrund: Diabetes typ 2 är en kronisk sjukdom som behöver bemötas dagligen. En av hörnstenarna i behandlingen är egenvård som personen själv utför vilket är nödvändigt för att bromsa eller förhindra komplikationer. För att personer med diabetes skall kunna utföra egenvården behövs kunskap om sjukdomen. Hur det är att leva med diabetes är en subjektiv upplevelse och behöver bemötas utifrån det. Människor kan uppleva det mödosamt att bryta vanor och beteenden, det är lätt att falla tillbaka till gamla vanor. Vården har ett ansvar att främja patienters hälsa och välmående och att ge verktyg för att sköta om sin diabetes.

    Syfte: Syftet med litteraturstudien är att belysa hur sjukvården kan stötta personer med diabetes typ 2 för bättre kontroll av diabetessjukdomen.

    Metod: Metoden för denna studie har varit litteraturöversikt. Ett utbrett sökande av vetenskapliga artiklar och efterföljande analys samt sammanställning har gjorts. De elva artiklarna som valts ut har varit av både kvalitativ och kvantitativ karaktär.

    Resultat: Författarna har kommit fram till tre teman. Det första temat är Utbildning och har tre subteman; Egenvårdsutbildning, Personcentrerad utbildning, och Utbildning i grupp. Det andra temat är; Motiverande samtal. Det tredje temat är; Skapa relationer och har tre subteman; Avsätta tid och lyssna, Personligt bemötande och Engagemang.

    Diskussion: Litteraturöversikten visar på att egenvårdsutbildningar ger viktiga kunskaper så att personer med diabetes tar riktiga beslut och utför egenvård korrekt. Vidare är relationen mellan sjuksköterskan och patienten viktig vilket även styrks av Dorothea Orems egenvårdsteori. Närstående kan också ha en positiv inverkan på egenvården om de är engagerade i den.

    Abstract [en]

    Background: Breathlessness is a symptom that makes breathing difficult which can lead to experiences of anxiety and fear. It is one of the most common symptoms experienced by patients with advanced stage disease. Several diseases such as COPD, cancer and heart failure can cause breathlessness. Palliative care aims to alleviate suffering by treating and preventing symptoms, such as breathlessness. Nurses need to be aware of patients´ experiences in order to provide and individually adjust symptomatic relief.

    Aim: Describe patients´ experiences of breathlessness, while receiving palliative care or receiving care for an advanced disease, including their strategies for managing breathlessness.

    Method: A literature review was carried out with data collection from five databases that resulted in 12 scientific articles of which 11 were qualitative and one was quantitative. The data analysis was made with Friberg (2017) method and resulted in two themes.

    Results: The two main themes that emerged were Experiences of breathlessness and Strategies for managing breathlessness. The result showed that breathlessness can cause anxiety and fear, physical fatigue and severe coughing. In addition, breathlessness implied limitations of patients´ daily life and could lead to patients being isolated in their homes, intended or unintended. Different strategies for managing breathlessness are presented.

    Discussion: In the discussion the pros and cons regarding the authors' joint work, challenges and considerations were addressed. The main findings of the result were discussed using Sister Callista Roy’s adaptation model.

    Abstract [en]

    Background: Mental illness is nowadays considered a public health problem. Patients with mental illness often suffer co-morbidity and are therefore often cared for within somatic care. According to caring science the holistic perspective of the human with both body and soul is advocated where one part can never be excluded. Sensitivity in the caring relationship can be crucial for a patient suffering from mental illness. However, health personnel themselves experience a lack of knowledge caring for patients with mental illness.

    Aim: The aim of the literature review was to illustrate patients´ experiences of being cared for within somatic care in conjunction with mental illness.

    Method: Literature review including ten scientific articles was implemented, eight with a qualitative design and two with a quantitative design. The articles were analyzed according to Friberg’s (2012) method for literature review.

    Results: Six main themes were identified: experiences of meeting health personnel, experiences of being understood, follow-up by the caregiver, experiences of being involved, experiences of barriers for care and security.

    Discussion: The result is discussed according to Eriksson's theory of the caring relationship and suffering. Lack of knowledge within mental illness creates insecurity among health personnel and also creates suffering for the patient. The importance of seeing to the human as a whole is discussed.

    Dose-response relationships of the sigmoid for urgency syndrome after gynecological radiotherapy.2018In: Acta Oncologica, ISSN 0284-186X, E-ISSN 1651-226X, Vol. 57, no 10, p. 1352-1358Article in journal (Refereed)
    Abstract [en]

    PURPOSE: To find out what organs and doses are most relevant for 'radiation-induced urgency syndrome' in order to derive the corresponding dose-response relationships as an aid for avoiding the syndrome in the future.

    MATERIAL AND METHODS: From a larger group of gynecological cancer survivors followed-up 2-14 years, we identified 98 whom had undergone external beam radiation therapy but not brachytherapy and not having a stoma. Of those survivors, 24 developed urgency syndrome. Based on the loading factor from a factor analysis, and symptom frequency, 15 symptoms were weighted together to a score interpreted as the intensity of radiation-induced urgency symptom. On reactivated dose plans, we contoured the small intestine, sigmoid colon and the rectum (separate from the anal-sphincter region) and we exported the dose-volume histograms for each survivor. Dose-response relationships from respective risk organ and urgency syndrome were estimated by fitting the data to the Probit, RS, LKB and gEUD models.

    RESULTS: The rectum and sigmoid colon have steep dose-response relationships for urgency syndrome for Probit, RS and LKB. The dose-response parameters for the rectum were D50: 51.3, 51.4, and 51.3 Gy, γ50 = 1.19 for all models, s was 7.0e-09 for RS and n was 9.9 × 107 for LKB. For Sigmoid colon, D50 were 51.6, 51.6, and 51.5 Gy, γ50 were 1.20, 1.25, and 1.27, s was 2.8 for RS and n was 0.079 for LKB.

    CONCLUSIONS: Primarily the dose to sigmoid colon as well as the rectum is related to urgency syndrome among gynecological cancer survivors. Separate delineation of the rectum and sigmoid colon in order to incorporate the dose-response results may aid in reduction of the incidence of the urgency syndrome.

    Abstract [en]

    This qualitative study aims to explore the status of gender equality and to provide background to the extensive sexual violence in India from the male perspective through the eyes of seven Gujarati men. The brutal rape of a young student, by whole world known as Nirbayha in December 2012 in the Indian capital, was the tragic event that started a mass reaction in India against the extensive sexual violence and has put the whole worlds light on the unequal conditions of men and women in the country. Being sexually abused as a woman is one extreme expression for female subordination in relation to men, accordingly “men´s violence to women” is a most current topic under social work research today. Social constructions and social structures need to be considered when studying sexual violence as well as gender issues.

        This thesis is based on a four weeks minor field study in Mandvi in Gujarat, India, where I conducted the interviews. The result is based on answers towards my main research question How do the interviewed men in Mandvi perceive gender equality, and what do they think of sexual violence?

    The answers clearly show the gendered thinking of the interviewed men. In the analysis I have tried to interpret and understand the data mainly based on the secondary research questions Are there any perceived connections between sexual violence and gender equality, and if so, how do these look? How do the interviewed men´s perceptions look in relation to earlier research and theoretical frames?  What can be done, in a place such as Mandvi on different societal layers (individual, family, society) to prevent sexual violence? In the analysis I show the data through the mirror of theoretical frameworks and the earlier research, such as social work theory, critical theory, gender theory and contemporary Indian masculinity research.

    Den svenska fattigvårdens, socialvårdens och socialtjänstens ”värdehistoria” handlar omhur mellanmänskliga värden, med religiösa och existentiella dimensioner, har använtsför att bygga upp, förvalta och utveckla offentliga lösningar. Etiska frågor har berörts påolika nivåer. Officiella texter om fattigvården, socialvården och socialtjänsten har främstavsett värden på samhällsnivån och medborgarnivån. Den professionella nivån har sällanberörts direkt.Sammantaget har socialtjänstens människosyn byggt på värden som gäller den enskildesindividualitet, möjligheter och behov med ett starkt underliggande fokus på individuelltoberoende. I dag finns det dock tecken på skillnader i värden mellan olika grupper,såsom rättigheter för barn och människor med funktionsnedsättningar och en mer traditionelltetisk värdegrund för äldreomsorgen.Trots att värdefrågor varit grundläggande och ofrånkomliga i officiella texter omfattigvård, socialvård och socialtjänst, är det ovanligt att analyser, reflektioner och ställningstagandeni lagarnas förarbeten direkt beskrivs som etiska. Det gäller både i äldre tidoch under de senaste decennierna. En gemenskapsmoral med rötter i akademisk filosofihar dock varit viktig för utformningen av de övergripande målen i socialtjänstlagens,SoLs, portalparagraf. I socialtjänstens samlade värdeuppsättning förenas en outtaladkonsekvensetik med drag av en kvalitativ utilitarism och spår av en äldre pliktetik.Det finns en risk för att etik används på ett mer retoriskt än analytiskt sätt i förarbeten.Det vilar därför ett stort ansvar på statens företrädare att visa omsorg om de värdensom genom lagar, förarbeten och andra texter förs vidare mellan olika nivåer inomsocialtjänstområdet.

    Bakgrund: Ökad global migration ställer krav på sjuksköterskor i omvårdnadssituationer där patientens etnicitet och sårbarhet behöver beaktas. Sårbarheten kan bestå i flyktingars hälsosituation och den stress flyktingar upplever under migrationsprocessen. Sjuksköterskor och sjuksköterskestudenter behöver ta kulturen i beaktande vid möte med dessa patienter.

    Syfte: Att belysa sjuksköterskors och sjuksköterskestudenters kulturkompetens.

    Metod: Litteraturöversikten grundar sig på tio vetenskapliga artiklar, varav två kvantitativa och åtta kvalitativa.

    Resultat: Sjuksköterskor och sjuksköterskestudenter finner kommunikationsproblemet som största hindret, där språkbarriärer är en tydlig faktor. Detta gör att sjuksköterskor och sjuksköterskestudenter anser att omvårdnaden i dessa fall tar mer tid i anspråk, och att tolkanvändningen förekommer alltför sällan. Sjuksköterskor och sjuksköterskestudenter anser att de har en viss kulturkompetens men att en önskan finns om ytterligare utbildning i ämnet.

    Diskussion: En önskan hos sjuksköterskor och sjuksköterskestudenter om fördjupade kunskaper i kulturkompetens kan utläsas av de resultat som framkommit i litteraturöversikten. Kulturkompetens har i litteraturöversikten diskuterats utifrån Madeleine Leiningers transkulturella omvårdnadsteori.

    Abstract [en]

    Background:

    Every year more and more people are affected with cancer and numerous people have many thoughts, concerns and feelings associated with both the disease itself and the provided treatment. Radiation therapy is one of the most common forms of treatment for cancer today. Radiation will almost always mean that the patient is forced to undergo a number of more or less unpleasant side effects to become free from their   cancer. There is therefore a risk that the patients health and wellbeing becomes inferior. That is why it is important to gain a deeper understanding of the patients situation during and after radiotherapy so that we as nurses should be able to provide good care.

    Aim:

    The aim of this study was to describe the experience in connection with radiation therapy for people with cancer.

    Methods:

    Literature Review by Friberg's method (2012) where 11 articles with qualitative design were analyzed for the results.

    Results:

    Three main themes emerged: Experience of the care environment, Whole life is affected and Social isolation and interpersonal   relationships. The environment of the radiotherapy room was perceived as frightening and claustrophobic. The patients lives were affected in form of   physical and psychological consequences. Fluid and nutrition problems dominated by radiation of the head and neck, leading to malnutrition and weight loss. Most of the participants experienced social isolation, either because of the isolation in the treatment room or they did not want to meet people because of the physical symptoms that occurred.

    Discussions:

    The result that meant physical, psychological and social consequences of radiotherapy is related to Katie Eriksson's concept of suffering; disease suffering, care suffering and suffering of life is discussed in relation to the main themes that emerged.

    Abstract [en]

    Introduction: Mental illness and suicide in young people from 15-24 years has increased in Sweden. To examine methods which can help in the fight against mental illness feels crucial, when traditional methods not always is sufficient for the target audience and for treatment of mental illness in youths. Attachment-based family therapy, ABFT is used for young people with mental health problems such as depression, anxiety and suicidality problems. ABFT is used today in child and youth psychiatry (BUP), in social services treatment for youth and in some state institutions for youth (SIS) in Sweden.

    Research questions: The study's purpose is to investigate psychotherapists, patients and parents different experiences of ABFT treatment. Interviews were conducted with four psychotherapists, two patients and two parents. Qualitative method was used and data collected have been analysed using thematic analysis of inductive approach.

    Results: The results are tied to five themes ; Attachment , relationships, communication, affects and change. Psychotherapists view re-attachment as necessary in order to re-establish dialogue and communication between parents and youth. This improves in the long run the relationship and affiliation, but also the mental state of youth according to psychotherapists. Young people's experience is that they became better understood and heard by their parents. The dialogue resumed and feelings could be expressed to the terms. When needs and feelings were received by the parent reliance grew on parents, the relationship improved and parents could be seen as a resource. Parents experience is that they got other ways to communicate, support and face his youth on.

    Discussion : Shows that reconnection and relational reformulation of the problems are made in the ABFT. The relational problems are consequences of the previous attachment injuries. Restored dialogue allows for reconnection, which improves the relationship. Extension of the fountain of youth and parent's capacity for emotional processing is the prerequisite for change. The expression of feelings associated with related injuries seems healing for young people as well as improves relationship and connection to the parent. Change occurs when youth put into words and share feelings with parents. To be received in their emotions creates a change in the internal working models of youth and parents. Changes occurs in the interaction between parents and youth which does that the internal working models of self and others can be reconsidered.

    The study's aim was to increase knowledge about the resistance of the Muslim religious expressions in the public sphere and its emergence on the basis of the case around the opposition to call for prayer. The study was conducted through the use of the qualitative research approach through interviews with informants who have a pronounced aversion regarding the existence of the public call to prayer in Sweden. The study's questions have been answered by an analysis of the empirical material by using different theoretical points that illustrate how Western standards are valued higher than the other, in the various standard systems. The view of the Muslim religious practices depicted by informants through an interpretative framework that is characterized by Islamophobic arguments that are considered to be incompatible with Western culture and the principles of the secular state striving. Such discourses can create an uneven distribution of power between Muslims and non-Muslims on the basis of the West's representations of Orientalism explained based in ethnocentric knowledge. The results show that these values are derived from the Islamophobic discourse and the negative perception of the Orient that creates a hierarchical system of norms on the basis of how Muslim religious practices understood and presented. The definition of religious freedom can be described as ambiguous, which presents an opportunity for individuals to interpret it based on subjective interpretations and knowledge of Islam is not objective constructed when religion is related to the negative phenomena and not from the Muslim experience. The study was intended to create an understanding in terms of how these interpretations are produced by non-Muslims and that contributes to a reduction in the Muslims' opportunities to exercise religious practices in the public sphere, where the call to prayer has been the trigger. This can contribute to Muslim social well-being and sense of solidarity that Swedish citizens is hampered, as these meet a resistance that can be explained on a structural as well as at the individual level through various social norms that prevail within different spheres of society and institutions. The right to exercise the call to prayer, according to the legal standards described as acceptable but questioned that right by social norms that society flourishes, creating a lack of understanding and an unwillingness of its existence in Sweden. The study is also intended to highlight the Islamophobic relevance of the research to other areas such as structural institutions and establishments by these forms of society and its individuals' social safety nets, where the social work plays a significant role, as previous research suggests a negative treatment of Muslims from social institutions.

    Abstract [en]

    Background: The nurse has a responsibility to through a person-centred and relation fostering approach support the patient's recovery process. The professional-patient relationship between nurses and persons with emotional instability within the closed psychiatric care is often characterized by frustration, irritation and misunderstanding, leading to an increased suffering. Brief admission as a method has the purpose to increase the patient’s sense of participation and autonomy, factors which are considered to improve the professional-patient relationship. The method has just recently been introduced in the Swedish health care system. There are however only a limited number of studies concerning brief admission in connection with emotional instability and deliberate self-harm, with no prior studies in Sweden.

    Aim: To describe nurses experience of the professional-patient relationship within brief admission among persons with emotional instability and deliberate self-harm.

    Method: The study was performed using the qualitative method. Eight nurses with experience from brief admission were interviewed and the data was analysed using a qualitative content analysis.

    Results: From the analysis two main categories emerged. The first category “Brief admission   promotes the professional-patient relationship” describes the experience of the interviewees that brief admission creates predictability and structure leading to improved   communication, continuity together with meetings between more equal partners. In the second category “Clarified role of the nurse” the interviewees describes a higher degree of focus on the care within brief admission together with working to support the resources of the patients, which affects the role of the nurses. 

    Discussion: The results of the study are illustrated and discussed using the tidal model and its ten commitments regarding the nurse’s professional responsibility within nursing together with relevant literature.

    Abstract [en]

    Background: Diabetes is an increasing problem and self-care is of great importance to prevent complications. Nurses have an important role in supporting or compensating in areas that the patient is not sufficient enough to self-manage. The interest in this topic originated in several clinical encounters with patients showing self-care deficits and tools for supporting these patients are in demand.

    Aim: The aim of this literature review is to illuminate patients’ experiences of barriers and promoters of self-care in type 2 diabetes.

    Method: Literature review. The result is based on 13 scholarly articles. The articles were identified from searching the databases Cinahl Complete, Academic Search Complete, PubMed and Nursing & Allied Health Database. This literature review focused on qualitative studies. A matrix method was used in compiling the results. The results were then analyzed and categorized.

    Results: Many factors affect individuals' ability of self-care. These are presented in the main themes: "Barriers" and "Promotors". Both internal and external factors influence a persons’ ability of self-care. A recurrent problem and barrier were lack of motivation and understanding of the importance of self-care. Coping, compliance and self-efficacy have a positive influence on self-care as demonstrated in the promotors theme.

    Discussion: Self-image and a persons’ emotional state affect his or hers self-care capacity and ability to change habits. Nurses need opportunities to improve their knowledge and understanding about lifestyle and diabetes counselling in order to help these patients. The results of this literature review were partially discussed using Dorothea Orem's "Self-care nursing deficit theory".
