    Abstract [sv]

    I denna uppsats har vi gjort ett försök att ta reda på hur det går till i samband med vidareutbildning för de som är anställda i Socialtjänsten. Vi har genom analys av enkäter försökt utskilja vad som varit särskilt hjälpsamt och vad som möjliggjort en god implementering av olika metoder. Vi har därför också sökt förståelse för dess motsats, nämligen de hinder som kan ha funnits.

    Vi hoppas att denna uppsats kan utgöra ett diskussionsunderlag för såväl studenter, socialarbetare och chefer, eftersom det kan vara bra att förstå processer av implementering och lärande inför att utbildningar ska planeras och genomföras.

    Vår avsikt med uppsatsen är inte att presentera nya teorier utan att försöka presentera hur upplevelser av implementering kan se ut för enskilda socialarbetare. Vi önskar också lämna ett kunskapsunderlag om implementeringens viktighet och slutligen vad vi sett som skulle kunna vara viktigt att beakta i detta hänseende.

    Det har på olika vis varit intressant att arbeta med uppsatsen, som inneburit att vi bland annat utformat en enkät som beskriver olika områden avseende implementeringens möjligheter och begränsningar. De nio informanternas reflekterande svar har bland annat visat att tiden är en viktig komponent att beakta då ett implementeringsarbete ska göras. Genom analys med hjälp av olika teoretiska utgångspunkter har vi slutligen kunnat presentera nya frågeställningar som kan ge uppslag till nya uppsatser eller forskning. Det har varit lika kul att få bearbeta och förstå informanternas många svar på frågorna samt se vart de leder.

    Abstract [en]

    Background: Throughout the ages sexuality has been a taboo subject. Today, knowledge is more extensive and the view on sexuality has developed into being less of a taboo but nonetheless, sexuality is fairly new and unexplored in care situations. One of the nurse’s primary tasks is to promote health and to prevent illness and to that the sexual aspect of health is also included. Research has shown that a large proportion of people who are cared for because of cancer, encounter obstacles or issues of sexuality or sexual dysfunction. Studies have shown that information regarding possible side effects on sexuality stemming from treatments and medications used against cancer, is scarce.

    Aim: The aim with this review was to illuminate the communication between health care professionals and adults in oncology care about sexuality.

    Method: This literature review was compiled by collection of material from previous studies and other scientific literature for the chosen field of health sciences. The basis for the outcome consists of ten scientific articles on cancer, sexuality and communication. The articles were found in the databases CINAHL Complete and PubMed. The material was examined and analyzed according to Friberg's method. The result was summarized in two main themes with two subthemes respectively.

    Results: The result shows that people who have been diagnosed with cancer rarely had talked to healthcare professionals about, or been given insufficient information about, treatment and pharmaceuticals and what consequences for the sexuality these and the disease itself may imply. Both healthcare professionals and patients state that it can be difficult to talk about sexuality as there are barriers or factors that can prevent such conversations. An often mentioned barrier was time, where patients did not want to spend precious time for the staff with their "trivial" problems or that the healthcare staff themselves blamed it on a lack of time. The results also show that information on sexuality was a priority for those who suffered from sexuality related cancers despite the fact that sexuality-related problems can be experienced regardless of the type of cancer or treatment.

    Discussion: The conclusion is that using the right methods and materials, communication on sexuality can be facilitated to a large extent. BETTER and PLISSIT are two models that, with the right application, also can help patients to address issues related to sexuality themselves.

    Abstract [en]

    On the first of January 1994 a new law was introduced in Sweden, Act on Support and Service for Disabled. The aim of the act was to ensure that people with disabilities got the help and support they need to live a normal life and to be able to live like others. Previous to this act there was great differences in support given to people with disabilities between the municipalities. The act on Support and Service for Disabled was meant to reduce these differences and ensure that people with disabilities got the help and support they needed, independently on where in Sweden they are living. Despite these aims the difference between the municipalities remains. This study examine why these differences still remains and what may affect social workers in their decision-making. It is based on a literature review of five different studies that regard legal rights in administration of this law. The result of the study is that the differences can be traced back to vague definitions in the law and that social workers define the act differently. The result also shows that these subjective assessments lead to low predictability of the decisions under the act on Support and Service for Disabled.

    Utvecklingen inom äldreomsorgen har under de senaste decennierna lett till en mer marknadsanpassad äldreomsorg. Den äldre ska efter att ha fått sitt biståndsbeslut välja vilken utförare som ska tillhandahålla den beviljade insatsen. Valfrihetssystem blir allt vanligare i samhället och valfrihet i sig har länge varit ett politiskt mål. Syftet med denna uppsats var att undersöka äldre personers erfarenheter av att välja hemtjänst. Frågor som har tagits upp är hur man väljer, vad som påverkar hur man väljer, om valet anses vara lätt och om man generellt är nöjd med det system som finns. Metoden som har använts är strukturerade intervjuer där respondenternas svar har kodats i SPSS. Åtta av intervjuerna spelades in och transkriberades. Uppsatsen har ett brukarfokus och utgår från de äldres perspektiv och syftar till att lyfta fram deras åsikter och erfarenheter om valfrihetssystemet inom hemtjänsten. Materialet har analyserats med hjälp av Hirschmans teori om exit, voice och loyalty samt med en deskriptiv beslutsteori, prospektteori. Uppsatsen kommer fram till att systemet idag inte är verksamt i den mening det var tänkt. Alla äldre väljer inte hemtjänst, fysiska och psykiska hinder står i vägen och få väljer att utnyttja sin rätt att byta utförare. Generellt är de äldre nöjda med den hjälp de får och valet av hemtjänstutförare är inte det primära för de äldre.

    Abstract [en]

    The aim of this study was to explore metaphors for discovering values and norms held by nurses in home-based nursing care. Ten interviews were analysed and interpreted in accordance with a metaphor analytical method. In the analysis, metaphoric linguistic expressions and two entailments emerged, grounded in the conceptual metaphor 'home-based nursing care is an endless journey', which were created in a cross-domain mapping between the two conceptual domains of home-based nursing care and travel. The metaphor exposed home-based nursing care as being in constant motion, thereby requiring nurses to adjust to circumstances that demand ethical maturity. The study focuses on the importance of developing further theories supporting nurses' expressions of their experiences of everyday ethical issues.

    Abstract [en]

    This essay intervenes in the politics of urban segregation in Stockholm. The main aim of the essay is to analyze and describe how advanced marginality and territorial stigmatization are expressed in the lived experiences of four unemployed youths, that have participated in a labor market training program located in the “social vulnerable” area Rågsved. With a theoretical framework based in Henri Lefebvre’s production of social space the empirical findings are interpreted in regard to how the youths produce social space in dialectical interplay with urban politics, advanced marginality, territorial stigmatization, and their local neighborhood. The empirical material in the study was collected through qualitative interviews with the youths. The method of interviewing, analyzing, and presenting the result is grounded in a phenomenological approach.

    The historical background for the essay is the politics of urban segregation that has emerged in the metropolitan areas of Sweden. The post-industrial society and advanced sectors in the economy are transforming the labor market, city landscape, and the requirements on workers. This deep transformation process has resulted in social exclusion and inequalities between different groups in the urban city. Unemployment and poverty has been concentrated to the urban periphery of the metropolitan city. The urban periphery is marginalized areas with high concentration of immigrant residents with post-colonial status. The Swedish Metropolitan Committee committed a proposal 1998 for a new urban politics in the beginning of the millennium, which main purpose was to intervene in the ongoing process of ethnic and socioeconomic segregation in the urban landscape. This political-institutional background, within the context of post-industrial society and neoliberal politics, situates the historical framework for the present study.

    The result in the study points out that the youths are in an insecure position in the contemporary labor market, and constantly reflect their ways of living through the dominated norms of active labor market policies. The youths participation in the labor market training program Rågsved Community Center are described as a positive experience, and they describe how they are fully recognized as individual subjects of the employees. Within the geography of urban segregation, the youths are constantly in a process of mental negotiating about how to determine the space of Rågsved. The space of the “social vulnerable” suburb Rågsved is produced by the youths in the conflict of territorial stigmatization and their feelings of belonging. 

    The main result from the study is that the youths lived experiences of participation in Rågsved Community Center reflects a political need for something different. They discredit the way that Arbetsförmedlingen approach them as unemployed and lack confidence in their methods. In the social space of Rågsved Community Center the youths are recognized as individuals and are also taking initiative to help their friends in Rågsved to find a way out of unemployment. These spatial practices in Rågsved produce a social space and constitute a local institution that could be an embryo for collective representation and organization in relation to urban segregation and youth unemployment in the urban periphery.

    Abstract [en]

    Background: The background describes aspects of the nurse’s profession and care responsibility within both general care and in dependent care. Furthermore, it describes the patient group with its problems, the concept of co-dependency and how it can impact the choice of nursing.  

    Aim: The aim of this study has been to explore nurses’ view of what characterizes good care and co-dependency within the closed depending care.

    Method: This study has a qualitative design with an inductive approach. Seven nurses at a   depending care unit in Stockholm were interviewed using an unstructured method. The material was then analyzed using qualitative content analysis.

    Results: The result from the interviews developed into three categories: The nurse’s view of what   constitutes good care within depending care, The view of the phenomenon of co-dependency within depending care and finally, The nurse’s perception of what differentiates good care and co-dependency. The result reveals, among   other things, the fact that while the nurse, intellectually, had no major problems in separating the concepts of good care and co-dependency, in practice it proved to be harder to implement.

    Discussions: The results are discussed in the context of previous research, other relevant literature and from Ida Jean Orlando’s interaction theory. In the discussion it reveals, among other things, how the nurses responsibility of care can develop into co-dependency.

    Abstract [en]

    The study aimed to seek understanding and knowledge about care managers professional role in elderly care. Whit that basis we went in-depth of the difficulties that can occur during the process of care managing and what kind of support resources that are available. We used qualitative research approach and performed six semi structured interviews with care managers in the elderly care in different municipalities in the county of Stockholm. The result was analyzed with the basis of theoretical reference frames that illustrate different kinds of dilemmas on both an individual and a structural level. The theories we used were Lipskys theory about street level bureaucrats, consequence ethics, virtue ethics and the concept of real dilemmas and deceptive dilemmas. The study shows that there are a number of difficulties that care managers face in their everyday work environment. Furthermore it emerged that care managers experience that they are too controlled by guidelines and laws which would therefore diminish their influence. The study is verifying of earlier research made in the same field.

    Abstract [en]

    Purpose and aims: This is the first study investigating sexuality from 6 months up to 7 years after burn. The aim was to examine sexuality in females and males by using the BSHS-B sexuality subscale and to examine possible contributing factors with regard to sociodemographics, burn characteristics, personality traits, and previous psychiatric disorders.

    Methods: A cohort of 107 patients consecutively admitted to a Swedish national burn center was followed up at 6, 12, and 24 months after burn, and 67 individuals were followed up at 2-7 years after burn. The present study utilized the BSHS-B sexuality subscale, and multiple regression analyses were used to examine possible contributing factors.

    Results: Women were less satisfied than men, and sexuality mean scores improved over time, even up to 7 years after-burn, in both men and women. The strongest contributing factors for worse outcome regarding sexuality were a history of psychiatric morbidity, neuroticism and burn severity.

    Conclusions: As some patients experience sexual problems after burns, even many years later, it is important to identify these individuals. The BSHS-B sexuality subscale may be used as a screening tool, but more in-depth assessment might be needed to address all aspects of sexuality.

    Abstract [en]

    Objective: To examine factors predicting psychiatric morbidity, taking into account the full range of psychiatric disorders before and after burn injury.

    Methods: A cohort of 107 patients consecutively admitted to a Swedish national burn center was examined for lifetime psychiatric morbidity, as well as 94 patients at 1 year postinjury. Sixty-seven individuals, some from that same cohort, were interviewed at 2 to 7 years postinjury. The predictive effects of psychiatric history, personality and other risk factors for psychiatric morbidity following burn were evaluated with multiple regression analyses.

    Results: The prevalence of having a psychiatric disorder preburn was 57%. One year postinjury 19% had minor or major depression and 23% had subsyndromal or full posttraumatic stress disorder. At 2 to 7 years, 31% fulfilled the criteria for a psychiatric disorder. The strongest contributing factors were a history of psychiatric morbidity and neuroticism.

    Conclusions: Two-thirds of the patients had a lifetime psychiatric disorder, and one-third had a psychiatric diagnosis 2 to 7 years postburn. Mental health problems can have a major impact on daily life and functional abilities. Thus, identification and treatment of a range of psychiatric disorders, taking into account preburn psychiatric disorders and personality, is important for optimal adjustment after burn.

    Bakgrund: Humor är ett komplex begrepp som färgas av den subjektiva upplevelsen och är föränderlig över tid. Tidigare forskning visar på att humor har använts som en copingmekanism för patienter med cancer, för att distansera sig från sin tillvaro och diagnos. Även sjuksköterskor har använt sig av humor för att skapa relation med patienten. Humor har använts för att hjälpa patienterna att hålla humöret upp genom att skifta deras fokus.

    Syfte: Syftet med denna litteraturöversikt var att belysa humor i sjuksköterske- och patientrelationen inom cancervården.

    Metod: Denna uppsats är en litteraturöversikt där innehållet av sju kvalitativa och tre kvantitativa vetenskapliga artiklar analyserades.

    Resultat: Tio artiklar analyserades där tre huvudteman framkom: Humor i sjuksköterske- och patientrelationen belyser att en relation mellan parterna bör finnas innan humor introduceras. Humor som ett omvårdnadsmässigt dilemma belyser när humor är tillämpbart och när det inte är accepterat. Positiv attityd och humor belyser vikten av en positiv attityd hos patienten för att kunna uppskatta och vara humoristisk.

    Diskussion: Diskussionen baseras på litteraturöversiktens resultat där det framkom att en etablerad relation mellan sjuksköterskan och patienten bör finnas innan humor kan användas. Oftast är det patienten som initierar humorn i relationen och det kan ses som en indikator på när humor kan användas. Åsikterna går isär kring huruvida om det är humor som skapar en positiv attityd eller tvärtom. Utifrån Katie Erikssons teori diskuteras relationsbildning som ansning, humor som lek och hur leken sedan utmynnar i lärande.

    Abstract [en]

    This research focused on the life situation of older people, who had moved to a nursing home at the end of life, from the perspectives of the older people themselves, care managers and nursing staff. The thesis is based on an analysis of 446 care manager assessments and decisions, interviews with seven care managers, six older people and 28 nursing staff. The main fi ndings are: (I); that there was a statistically signifi cantly shorter waiting time for a move to a nursing home for older people who were in hospital compared to those who were living in their own home at the time of the decision. Seventy percent of the decisions made by care managers’ concerned women. The waiting period for men was fi ve days shorter compared to women. (II); that the care managers’ descriptions revealed that their assessments of the needs and wishes of the older people were infl uenced by whether or not it was clear that the older person had only a limited time left to live. The care managers’ way of reasoning has been conceptualised as two approaches, the medical and the natural path to death, where the former was characterised as fl exible and collaborative, whereas the latter was governed to a greater extent by a “wait and see attitude”. (III); that the older people’s experiences of living in a nursing home have been conceptualised into three themes: feeling like a stranger in an unfamiliar culture, being excluded from life, and living while waiting for death. The latter involved a deep insight that life would soon come to an end; a fact the staff appeared to take into account to only a minor extent. (IV); that dying and death was characterised by a discourse of silence, with tension between avoidance of and a confrontation with death. Staff members who expressed a fear of death held it at a distance by concentrating on practical tasks and avoiding close contact with older people who were dying. The thesis highlights the fact that the dying and death of older persons was characterised by a discourse of silence and several transitions. Death was not a topic that the staff members or older people generally talked about, and care in the fi nal phase of life was not actively or explicitly planned. In terms of access to a nursing home bed, only older people with an extensive need for care obtained such a place. These fi ndings imply that all older people can be said to be in need of palliative care. The older people in our study were in a liminal phase, and waiting for death. Feelings of social and existential loneliness and that their living space was shrinking were evident.     Nursing homes are a setting in which death and dying is common. How death and dying is articulated and the actions that take place in a nursing home constitute adiscourse that guides the staff in their work. The aim of this studywas to explore thediscourse of death and dying in nursing homes from the perspective and understandingof the staff. The study draws on Foucault’s discourse analysis. Data arefrom fivefocus-group discussions held with 28 staff of four different nursing homes in Sweden. The findings show that the discourse had three characteristics :(a) dying was silent and silenced, (b) emotions were pushed into the background,and (c) attentiveness to death arose after the moment of the elderly person’s death. The structure of the discourse was characterised by a movement between twopositions, avoiding and confronting death, themain focus being on avoidance. Thearticulation and practices of silence highlight a need to regard dying as a processthat requires attention. One way to ensure appropriate attention could be to instil the philosophy of palliative care in nursing homes, including training and supportfor the staff in their work. The study demonstrates that nursing-home staff needmore knowledge and support to enable them to feel that they do a good job.

    Abstract [en]

    Seven care managers employed by a large municipality in Sweden were interviewed concerning their reasoning regarding end-of-life care for older people. Data were analyzed using a hermeneutic approach. The results showed that end-of-life care was considered to constitute a small part of the care managers' work and was something they did not focus on in general when assessing care needs. Two different pathways to death--the natural and the medical--were identified. In the natural pathway, death was invisible and the care was more routine-oriented. In the medical pathway, death was visualised and the care more individualised. Neither of the pathways paid attention to communication or existential needs. Thus, there is a need for a palliative pathway to death based on the philosophy of palliative care, which could provide guidance for care managers and promote opportunities for older people to achieve a dignified dying and death.

    In Sweden, increased care in ordinary housing has contributed to a reduction of rooms in sheltered accommodation. The allocation of rooms has become stricter. Only those whose care needs cannot be met in any other ways are allocated such accommodation. The aim was to explore the waiting time between the transfer decision and the accomplishment of the move from the initial form of care to sheltered accommodation as well as whether there were differences in waiting time in relation to certain demographic data. METHOD: 445 decision documents were analysed. Mean and 95% confidence intervals (CI) for waiting time and date of the move to sheltered accommodation were calculated. Differences between mean age and waiting time were analysed using Student's T-test. Effects of age, gender and cohabitation on waiting time were estimated by means of multifactor linear regression. RESULTS: The main finding was that the difference in mean waiting time was shortest when moving from hospital, irrespective of destination. There were no significant differences in waiting time in relation to gender, age or cohabitation. CONCLUSION: The reason for a move was often described by means of abstract standard formulations. There is a need for standardised models and assessment instruments in order to ensure older people's safety and to compare different forms of accommodation.

    AIM: To describe Swedish nursing students' perceptions of caring for dying people after the first year of a three year in a nursing programme at three university nursing schools in Sweden.

    METHODS: Interviews (n=17) were undertaken with nursing students at the end of their first year. A phenomenographic approach was used to design and structure the analysis of the nursing students' perceptions.

    RESULTS: The analysis resulted in five categories: 1) from abstract to reality, 2) from scary to natural, 3) increased knowledge can give bad conscience, 4) time limits versus fear of end-of-life conversations, and 5) meeting with relatives.

    CONCLUSION: Nursing students need to be prepared both theoretically and within practice to encounter death and dying and to care for dying persons. By combining their theoretical knowledge of dying and death with their own encounters of death and dying people in practice, the students can be supported to develop an understanding of dying and death as a natural part of life rather than something frightening.

    Aim: The aim of the study was to deepen the understanding of how older persons living in a nursing home experience life close to death.

    Background: A move to and a life in a nursing home while being close to death is a reality for many older people in Sweden. Being able to express thoughts and feelings about death has been described as both crucial for sustaining personhood as well as for establishing a meaningful existence at the end of life. Important are the experiences of older people living in nursing homes who are approaching death.

    Method: Six older people were interviewed on one to four occasions. A total of 16 interviews were conducted with the participants. An interpretative approach was chosen.

    Findings: The main interpretation, Feeling lonely in an unfamiliar place, is based on three themes (i) Waiting for death, with the subthemes death as a release and thinking of oneself as dead; (ii) Subordinate oneself to values and norms of the staff, with the subthemes feeling offended and feeling trapped; and (iii) Keep the courage up. The older people's lives were characterised by feelings of aloneness in an unfamiliar place which contributed to a sense of existential loneliness. They experienced few opportunities to discuss their thoughts of life and death, including preparations for passing away.

    Conclusion and implication for practice: It is of importance for professionals to be able to meet older people as they are and respect them as human beings in their transitions, before, during and after the move to a nursing home. It is important to find ways to support older people's wellbeing and identity near death.

    The continuing growth of social media during the last decade has resulted in an increased interest in what affect it has on our romantic relationships. Existing research using quantitative methods has suggested a correlation between excessive use of social media and an increase in feelings of jealousy and loneliness. This qualitative study of interviews with five young adults aims to examine in what ways the use of social media can be seen affect romantic relationships. Beyond confirming earlier findings, the study shows that the effect of social media on romantic relationships primarily is as a magnifier of pre-existing relational problems and a catalyst for potential relational difficulties. Among those problems described by the participants are an increase in distrust and suspicion, an increased need for control/supervision of one’s partner together with the experience of competing with social media for the attention of the significant other. Furthermore, the participants experienced that the strength of an individual’s self-esteem plays an important role regarding the degree of likelihood with which these problems arise within the relationship. The study also shows how the possible effect that frequent social media usage may have on the relationship in the long term is a question that is given much consideration by the participants.

    In pace with changes in society, and as the number of youth with mental illness increases, the forms of help available to youth with mental health issues also develop and change. The evolution of the internet, among other things, has resulted in traditional forms of help, where the one meets a professional face-to-face, being supplemented with online forms of help, such as online chats. In view of this development, this study aims to examine and compare the attitudes of young individuals towards seeking help for mental illnesses from a counselor and an online chat. This thesis will be accomplished using a quantitative approach where paper questionnaires are used as a data collection method. A total of 223 questionnaires were answered by adolescents aged 15-20 years in the Stockholm area. The study’s results show that respondents feel more secure with and trust towards a counselor’s help and are also more likely to seek this form of help than online chat.  Responses varied in regards to what affected attitudes toward each form of assistance, but ease of access, trust and the medium of communication was reported to be the main factors affecting willingness to seek the help of a counselor, while lack of trust and stigma were the main factors contributing to reluctance. The respondents who were willing to seek help from an online chat stated however that accessibility and anonymity were primary factors. The majority who could not imagine seeking such online assistance stated lack of confidence and awareness as well as medium of communication as the foremost explanations. The study also showed that respondents are not only more willing to seek help for mental illness from a counselor than through online chat; they also have more experience in seeking this type of support. The factors that respondents felt to be positive and/or negative with their experience differed considerably from those factors that influenced the respondents' attitudes toward seeking help from a counselor or an online chat. Respondents drew attention to factors like the professionals' attitude and whether they received help with his or her problem as factors that characterized their experiences. Generally, however, the majority of respondents’ had positive experiences of seeking help from both counselors and online chats.

    Syftet med vår uppsats är att öka kunskapen om anhöriga till äldre immigranter såsom omsorgsgivare, i uppsatsen kallade ”sent-i-livet-immigranter”, med särskilt fokus på upplevelser om valmöjligheter ifråga om omsorgsrollen.  Vi var intresserade av hur det kommer sig att anhöriga går in i en omsorgsroll i förhållande till den äldre och vilka andra formella/informella omsorgsgivare det finns kring honom eller henne. Slutligen var vi intresserade av att ta reda på om anhörigvårdare anser att de har en valmöjlighet att gå ur/lämna sin omsorgsroll till någon annan om det skulle vara deras önskan.

    Vi har utfört fem semistrukturerade intervjuer med anhöriga till sent-i-livet-immigranter där de har en betydande omsorgsroll. Vår metod utgick ifrån en kvalitativ och induktiv forsknings ansats. Vi analyserade materialet med hjälp av tidigare forskning samt utifrån det teoretiska begreppet skyddandets förnuft samt den s. k. coping-teorin.

    Studiens resultat visar att våra informanter går att kategorisera i två olika grupper: de anhöriga som gjort ett mer aktivt val och de som gjort ett mindre aktivt val att ta hand om sin äldre. Vi har även kunnat urskilja fyra olika teman som återkommer bland våra informanters svar: synen på den offentliga omsorgen, vi och de, hinder och tillfredsställelse och hur de ser på valet att gå in och ut ur en omsorgsroll. Vi har även med våra resultat kunnat bekräfta det teoretiska begreppet skyddandets förnuft då vi sett att – oavsett om våra informanter gjort ett mer eller mindre aktivt val – samtliga informanter har skyddandet av sina äldre gemensamt.

    OBJECTIVES: To determine whether chronic treatment with coenzyme Q10 or remacemide hydrochloride slows the functional decline of early Huntington's disease (HD).

    METHODS: The authors conducted a multicenter, parallel group, double-blind, 2 x 2 factorial, randomized clinical trial. Research participants with early HD (n = 347) were randomized to receive coenzyme Q10 300 mg twice daily, remacemide hydrochloride 200 mg three times daily, both, or neither treatment, and were evaluated every 4 to 5 months for a total of 30 months on assigned treatment. The prespecified primary measure of efficacy was the change in total functional capacity (TFC) between baseline and 30 months. Safety measures included the frequency of clinical adverse events.

    RESULTS: Neither intervention significantly altered the decline in TFC. Patients treated with coenzyme Q10 showed a trend toward slowing in TFC decline (13%) over 30 months (2.40- versus 2.74-point decline, p = 0.15), as well as beneficial trends in some secondary measures. There was increased frequency of nausea, vomiting, and dizziness with remacemide and increased frequency of stomach upset with coenzyme Q10.

    CONCLUSIONS: Neither remacemide nor coenzyme Q10, at the dosages studied, produced significant slowing in functional decline in early HD.

    BACKGROUND: Riluzole retards striatal glutamate release and pathologic consequences in neurotoxic animal models of Huntington's disease (HD).

    OBJECTIVE: To determine the dosage-related impact of riluzole on chorea in HD.

    METHODS: An 8-week double-blind dose-ranging multicenter study of riluzole was conducted in 63 subjects (32 women, 31 men) with HD who were randomized to receive placebo, riluzole 100 mg/day, or riluzole 200 mg/day. The prespecified outcome measure was change in the total maximal chorea score of the Unified Huntington's Disease Rating Scale (UHDRS).

    RESULTS: Fifty-six (89%) subjects completed the study. A reduction (p < 0.01) in chorea at 8 weeks was found using a linear trend test with dose. Comparing the groups individually, the reduction in chorea for the riluzole 200-mg/day group (-2.2 +/- 3.3) was different (p = 0.01) from placebo (+0.7 +/- 3.4), but the riluzole 100-mg/day group (-0.2 +/- 2.9) was not. Riluzole did not improve other motor, cognitive, behavioral, or functional components of the UHDRS. Alanine aminotransferase was elevated in a dosage-dependent fashion (p = 0.01).

    CONCLUSIONS: Over 8 weeks of treatment, riluzole 200 mg/day ameliorated chorea intensity in HD without improving functional capacity or other clinical features of illness. Riluzole 200 mg/day was attended by reversible liver transaminase abnormalities that would require monitoring in long-term studies.
