Abstract [sv]

Bakgrund: Åttio procent av alla som avled i Sverige 2024 uppskattades vara i behov av palliativvård; globalt estimerades 56,8 miljoner människor ha samma behov. Palliativ vård är en patientcentrerad vård som ämnar att lindra lidande och främja livskvalitet för personer där döden väntas inträffa under de kommande dagarna, veckorna, månaderna eller åren. Palliativ vård innebär inte nödvändigtvis att döden är nära, men när patienter befinner sig i livets slutskede ges palliativ vård för att möta de fysiska, psykiska, sociala och existentiella behoven som finns när en person närmar sig döden. Sjuksköterskans roll i den palliativa vården utgår från fyra hörnstenar: symtomlindring, teamarbete, kommunikation och närståendestöd.

Syfte: Syftet med denna litteraturöversikt var att undersöka sjuksköterskors erfarenheter av att vårda patienter i livets slutskede inom olika vårdkontexter.

Metod: Den valda metoden för detta examensarbete var litteraturöversikt. Data har insamlats från tio originalartiklar som hämtats genom strukturerad databassökning i Cinahl Complete och PubMed med hjälp av sökord. Alla artiklar har kvalitetsgranskats enligt Fribergs (2022) 13 granskningsfrågor. Analys av data genomfördes tematiskt med färgkodning.

Resultat: Litteraturöversiktens resultat visade att vård i livets slutskede innefattade många olika, ofta negativa, aspekter som påverkade hur sjuksköterskor upplevde sitt arbete. Bland dessa aspekter var den emotionella bördan, mötet med patienter och närstående samt sjuksköterskornas egna kulturella bakgrunder och upplevelser av döden särskilt utmärkande.

Slutsats: Sjuksköterskor inom allmän palliativ vård behöver organisatoriskt stöd och få möjlighet att utöka sin kompetens i hur vård i livets slutskede ska ges och hur den emotionella bördan som arbetet kan orsaka bör hanteras för att minska risken för medkänslotrötthet.

Abstract [en]

Background: Eighty percent of recorded deaths in Sweden, 2024, were estimated to be in need of palliative care; on a worldwide scale, 56,8 million people had the same needs. Palliative care is a form of patient centered care which aims to relieve suffering and promote quality of life for people who are awaiting death in the coming days, weeks, months or years. Palliative care does not inherently entail end of life, but for people who are at their end of life, palliative care is needed to meet the physical, mental, social and existential needs often seen at this stage. The nurses’ role can be identified through the following four cornerstones of palliative care: symptom relief, team work, communication and support for families.

Aim: The aim of this literature review was to explore nurses’ experiences of caring for patients at the end of life within different care contexts.

Method: The chosen method for this study was a literature review. Data was collected from ten original articles obtained through a structured database search in Cinahl Complete and PubMed using keywords. All articles were quality-reviewed according to Friberg’s (2022) 13 review questions. Data analysis was conducted thematically with colour coding.

Results: The literature review highlighted many aspects, often negative, of how nurses experienced their work in treating patients at the end of life. The results showed the emotional burden on the nurses, meeting with patients and their families, as well as nurses' own cultural backgrounds and views of death as of particular importance.

Conclusion: Nurses in general palliative care need organizational support and the ability to increase their competence in how to give end-of-life care and how to lessen the risk for compassion fatigue as a result of the emotional burden they experience from their work.
